Project ApproachISF implemented a structured and managed project process that protected both applicant and evaluator identities while maintaining data integrity. ISF evaluated 206 MAT applications (on the CF-MH 4041 form) from 20 approved applicants against established DCF standards and requirements, as laid out in the CF-MH 4040 form. The evaluation scores were then digitally extracted and placed in an Excel workbook, including a summary pivot table, from which totals and averages for each application were calculated.To organize the teams, evaluate the applications, and extract the data, ISF implemented a project approach that:Established and onboarded 4 primary teams and 1 reserve team of evaluatorsDistributed the MAT applications amongst the 4 teamsPerformed evaluations based on DCF requirementsCollected, collated, and analyzed the evaluationsDeveloped an ordered listing of the MAT applications based on the evaluation team resultsAdditional project execution details are contained in Section REF _Ref34639993 \r \h 2 - REF _Ref34640005 \h Process.Executive SummaryThe 206 CF-MH 4041 MAT application forms were successfully reviewed against the DCF requirements. Applicant names were abstracted to unique applicant numbers to ensure evaluators were neither aware of nor biased by any preexisting opinions of a given applicant. Each of the 206 CF-MH 4041 forms was evaluated by 3 evaluators on a single evaluation team.All applications for a given county were assigned to a single evaluation team.All CF-MH 4040 evaluation forms were digitally signed by each evaluator using DocuSign and uploaded to a private, secure, and access-controlled SharePoint document repository. Individual criteria scoring data was extracted from each CF-MH 4040 evaluation form. This data was inserted into a consolidated Excel workbook. The subtotals for each criteria category and overall total scores for each application were calculated and presented in a summary pivot table. The summary scores are presented in Section REF _Ref35004056 \r \h 3.3 - REF _Ref35004068 \h Evaluation Scores by County.The consolidated Excel workbook is delivered as a separate file along with this document. The pivot table displays each applicant separated by their Florida county location and ordered by highest to lowest total score. This report presents both a summary of the evaluation process and the resultant metrics and sorted scores. None of the individual criteria scores were modified from the original evaluator’s CF-MH 4040 inputs.Evaluation Process SummaryIn the first phase of the Methadone Medication-Assisted Treatment (MAT) application evaluation review project, ISF assembled 4 primary and 1 reserve evaluation teams as outlined in the LD100 Project Management Plan, Section 2.1.3.1 - Evaluation Team Tasks. All 4 primary teams were subsequently contracted, on-boarded, and assigned an equitable share of the MAT applications for evaluation. During the recruitment process, ISF leveraged networks of highly qualified subject matter experts to select professionals that meet or exceed the qualifications required for each of the three evaluator areas as outlined by DCF. Each evaluator’s qualifications were provided to DCF as part of Deliverable 2 – Evaluation Team Tasks. The selection for each team included:1 Medical ProfessionalMedical Professionals include Medical Doctors, Psychiatrists, or Advanced Practice Registered Nurses in any specialty or general practices with:A minimum of 5 years’ experience in addictions medicine or treatment of substance use disorders, and An active medical license issued within the United States.1 Public Health Policy ProfessionalPublic Health Policy Professionals include persons with a minimum of 5 years’ experience implementing state-level public health policy addressing addictions medicine or treatment of substance use disorders within the United States.1 Academic ProfessionalAcademic Professionals include university faculty with an active teaching or research position in the field of addictions medicine or treatment of substance use disorders within the United States.ISF verified and documented that each evaluator had no conflict of interest with either the State of Florida or with the list of applicant organizations – provided by DCF and listed alphabetically in Table 1 below.Table 1 - MAT Applicant OrganizationsMAT Applicant OrganizationsAspire Health Partners, IncMeridian Behavioral Health IncBetter Choices Clinic LLCMetro Treatment of Florida LPCFSATC dba Central Florida Treatment CentersOperation Par IncCRC Health Treatment Clinics LLCPark Place Behavioral HealthDACCO Behavioral Health IncPsychological Addiction Services, LLCDISC Village IncRiver Region Human ServicesGolden Glades Treatment CenterRiverwood Group LLCLakeview Center Baptist HealthcareSMA Healthcare IncLifeStream Behavioral CenterSymetria Health of Florida LLCMaric Healthcare, LLCTreatment Centers of America To ensure each primary evaluator did not have a conflict of interest with the MAT applicant organizations, ISF executed and maintain the below items as part of the ISF personnel record for each primary evaluator: ISF standard contract agreement with non-disclosure statements signed by evaluators.DCF-provided conflict of interest statement - included in ISF contract agreements.Evaluator-provided confirmation of no conflicts of interest with applicant organizations.ISF completed internal due diligence reviews of each evaluator to ensure a conflict of interest did not exist.During the evaluation process, the 12 evaluators scored 206 MAT applications from the applicant organizations against established DCF standards and requirements (as defined in CF-MH 4040). The CF-MH 4040 was a fillable PDF form that allowed evaluators to enter scores and notes for each evaluation question. When each evaluation was completed, it was digitally signed by the evaluator, using DocuSign – an industry-standard and legally-binding digital signature (). Evaluators uploaded each completed and signed CF-MH 4040 to an ISF-managed access-controlled secure SharePoint document repository, segmented by each evaluator. Data integrity was maintained throughout the process by restricting access to this repository and moving evaluation forms to an extraction folder when completed. To eliminate potential manual transcription error during the transfer of individual criteria scores from the evaluator CF-MH 4040 forms, ISF developed a Microsoft PowerShell script to extract the evaluator data from every input field within the digitally signed CF-MH 4040 PDF into an evaluator-unique comma-separated-value file. Criteria subtotal scores and a summary total score for each application were calculated in Excel, based on those individual criteria scores. These 12 spreadsheets were imported into a consolidated Excel workbook, and a pivot table was created to allow for ease of data viewing and ordering.The individual criteria scores from the evaluation teams were checked for completeness, and the individual team member assessments were checked for quality reviews to ensure they adhered to the maximum point values for each section of the CF-MH 4040 assessment form. None of the evaluator individual criteria scores were modified.Detailed Evaluation ResultsThis section provides the detailed results from the evaluation process, including applicant submission metrics and the sorted total and average scores for each Florida county.Electronic Evaluation ScoresISF extracted the raw individual criteria scoring data from each CF-MH 4040 evaluation form, consolidated the data into a single Excel workbook, and developed a comprehensive pivot table displaying the 20 applicant organizations and their scores. The Excel workbook with this information is provided to DCF as part of this deliverable. As a layer of security, the individual and consolidated data worksheets are protected and locked. The password allowing sheets to be unlocked will be provided to DCF under separate cover. The pivot table is not protected or locked. A Guidelines tab is available within the workbook that provides information on the organization of the Excel workbook. Application MetricsThe tables below display the number of applications submitted for each county (Table 2) and the number of applications submitted by each applicant organization (Table 3).
