Ten Panel Urine Drug Screen Instructions
• Required only for students participating in pharmacy areas or if otherwise specifically requested
• Must be dated within the 6 months before your program start date
• Please submit copy of results with other documentation required
• LVHN does not provide urine drug screens for students
Please attach/submit the results of a 10 panel drug screen. Documentation of a negative 10 panel drug screen is required prior to your start date. This drug screen must be dated within the six months preceding the start of your educational experience. This type of drug screen can be obtained at any commercial laboratory service center. Some laboratories require a physician's prescription to perform the test, so please call ahead, and, if required, obtain a prescription from you primary care physician. Alternatively, if you are in the Lehigh Valley area prior to the start of your experience at LVHN, HealthWorks (addresses/phone listed below) provides drug testing services on a walk in basis, no prescription necessary. The student is responsible for any and all charges. The 10 drugs that we require you be screened for are as follows:

Drug screening can be administered at the location of the student’s choice (ie. private physician's office, school health center, or at any of the Health Works clinics listed above. Note: The student is responsible any and all charges. Charges may vary widely at different facilities – we suggest students request estimate of fees before choosing where to have tests completed.
