Homeless Prenatal Program (HPP)

Title: Assistant Program Manager KFT/FTC/DR

Reports to:    Program Manager

 

SUMMARY: This position is responsible for assisting in the oversight of Homeless Prenatal Program's Child Welfare and Family Treatment Court Programs (KFT/FTC/DR) staff, applicable contracts, and providing a level of direct service as determined by the Program Manager. This position will focus on the requirements of the Family Treatment Court (FTC) contract and the oversight of the Peer Mentor Training Program. This position will provide support for Family Case Managers as requested by Program Manager or Director of Programs.

 

Essential Duties and Responsibilities:

 

Supervision and Oversight:

• Ensures that all case management staff, regardless of program, participate in regular court hearings and provide all necessary documentation required, in collaboration with FTC.

• Ensure compliance with program policies regarding timely completion and submission of HSA Substance Use Disorder Assessments and HPP assessments, action plans, and dosage through regular dashboard review and tracking. In cooperation with Program Manager, follow-up with Case Managers on outstanding actions as needed.

• Ensures KFT/FTC/DR staff provides Treatment Progress reports to FTC.

• Oversee the case management of referred clients on an ongoing basis.

• Maintains ongoing collaboration with Child Welfare and treatment providers.

• Oversight of external Behavior Health Assessments including ensuring clients are referred to Treatment Access Programs and that Substance Use assessments are scheduled.

• Provide case management expertise and feedback to KFT/FTC/DR staff as well as assessing and supporting skill development needs for team members.

• Ensure that all Case Managers are trained and capable of addressing issues of housing, pregnancy, domestic violence, mental health, child welfare and substance abuse.

• Monitors calendars, appointments, dashboards and follow up of KFT/FTC/DR staff.

• Ensures that KFT/FTC/DR staff are aware of all contract objectives and outcomes as well as HPP initiatives and procedures.

• Conduct regular data integrity/quality review of Salesforce and files and follow up with staff when there are concerns or necessary corrections.

• Provide oversight to the Peer Mentor Program including recruitment, developing/documenting curriculum and timelines, and skill assessment and development of 1-2 Peers.

• Direct services as required.

 

 

 

Program Management and Administration:

• Attend program meetings set by Family Treatment Court, HSA and other participating agencies in regards to contracts.

• Responsible for assisting in maintaining communication with the Family Treatment Court staff and the Child Welfare Department in regards to contracts.

• Coordinates all referrals made by the Child Welfare Department and other programs.

• Enter FTC related activities, such as treatment plans and progress reports, into the FTC database as needed. Collect and distribute reports from the FTC database to HPP staff, as needed.

• Assist with tracking budget spending.

• Monitors FTC, HSA, FRC and any other contracts to ensure that contractual obligations are met and funding reports are completed in a timely manner.

• Participates in any scheduled contract evaluations.

• Provide administrative support for KFT/FTC/DR programs, including developing policies, client tracking, reporting, contract compliance and data integrity.

• Attends weekly Management Team meeting to address program issues.

• Support Program Manager in implementing established performance standards to make sure that agency policies and practices are upheld.

• Facilitate and/or assist at weekly Team Meeting and create Agenda.

• Represent HPP in the community.

• Other duties as required.

 

Qualifications

 

• Bachelor’s Degree/AA in related field preferred or at least three years comparable life/work experience.  

• At least two years case management experience/supportive management experience.

• Knowledge of, and/or experience in, substance use disorders required.

• CAADE or CCAPP certification strongly preferred.

• At least one year of supervisory experience required.

• Knowledge of, and/or experience in, Child Welfare/Child Protective Services preferred.

• Second language, especially Spanish, welcomed.

o Strong sense of self-awareness and sense of limitations.

o Appropriate professional and personal boundaries with high integrity.

o Knowledgeable about dynamics of homelessness, families, substance abuse, mental health, domestic violence, and parenting.

o Knowledge of existing services available for clients.

o Computer proficiency in Microsoft Word and Excel.

 

 

Physical Requirements:  This position requires the ability to work under stress and handle multiple project deadlines. This position requires an annual tuberculosis test and may require moderate lifting.  

 

Certificates, Licenses, Registrations:  California Driver’s license and insurance preferred.

 

Work Environment: Fast-paced, multi-cultural, collaborative work environment

 

Hours: Full time - 39 hours/week. Scheduled hours will be 9-5 M-Th and 9-4 on Friday.

Salary: DOE + excellent benefit package

FLSA Status: Non-exempt
