? Enter Registration Key APGNEW2017 and Click "Submit" Or log in as a "Returning Participant" with your username and password you established the first time you logged in.
? Then enter your Unique ID First Name, Last Name, first 4 date of birth and click "Submit" Twice.

3 Click "Continue" to schedule your appointment.
4 To visit a Quest Diagnostics Patient Service
Center (PSC) -- choose to Schedule an Appointment or register as a walk-in.
5 Choose PSC location and appointment time.
