In order to schedule an online appointment at a Quest Patient Service Center, please follow the instructions below:

1. After completing your Registration and the Know Your Number Health Assessment return to My Dashboard and click on the Quest Clinic Button or visit

2. Once you click on the Quest Clinic button you will access the Quest Diagnostics, My Blueprint to Wellness website.

3. Enter Participant Information - enter your last name and the last 4 digits of your Social Security Number, (example ? Smith1234) and your Date of Birth. Select "Yes" if you are a Schneider employee or "No" if you are the spouse of an employee and click Submit.

4. Review your pre-populated demographic information, create a username and password, and select a secret question & response.

5. Select CONTINUE under "Schedule Your Screening"

6. Review and agree to the Terms and Conditions of Service

7. Schedule an Appointment ? select Yes, I would like to schedule an appointment.

8. Enter the desired Zip Code in order to find a Patient Service Center within 50 miles.

? "Walk In Only" indicates appointment scheduling online is not available for that Patient Service Center, enter another zip code to locate a clinic with online scheduling or if you plan to attend as a Walk In proceed to log off. Please note you may experience long wait times as a Walk In appointment.

9. Continue to schedule online - Click "Schedule" on the right hand side of the location you wish to attend.

10. Select an available date and time

11. Review your appointment confirmation to ensure that you are scheduled. You will have the option to enter your email address to receive an email confirmation and reminder.

12. Print the confirmation page and bring it with you to the Patient Service Center along with a photo ID.

13. Remember to fast 10-12 hours for the most accurate screening results.
