Please return this form to participate in our parade along with your entry fee.

Commercial Groups (For Profit) ? $75 The $75 entry fee will be waived for some sponsors. Please visit our website at to see how your group can become a Dan Patch Days sponsor.

Non-Profit Groups ? $25 Please visit our website at or contact Anne Masis of the Savage Chamber of Commerce at 952-894-8876 to learn about the many opportunities available at this year's event.

In consideration of my or my child's participation, I agree that all activities undertaken by me or my child, are at my or his or her own risk, and that the Dan Patch Days Inc, its agents and volunteers, shall not be liable for any claim, demand, injury, damage, action, or causes of action whatsoever to myself or my child or my property or my child's property due to the passive or active negligence of the Dan Patch Days Inc, its agents, or volunteers, arising out of, connected with my or my child's participation in or the use of operation of equipment in this event and that I expressly forever release and discharge Dan Patch Days Inc, its agents, or volunteers from all such claims, demands, injuries, damages, actions or causes of action whatsoever. I understand that my signature is required before my or my child's participation in this event is allowed. I have read this Agreement carefully and know and understand its contents. I agree to participate, or allow my child to participate in the event sponsored by Dan Patch Days Inc. I understand that participation in the event is completely voluntary. The Dan Patch Days event may be photographed, and participants may be photographed for the use in promotions without approval or compensation.
