|Purpose of the Role … |

|The Case Manager is responsible for operational coordination and implementation of case management support for families in the Belconnen region, |

|including referral and networking. Case Managers will provide support and advocacy for vulnerable children, young people and families requiring |

|medium to long term assistance. Through assessment, planning, implementation and review individuals will be empowered to set goals and build |

|resilient lives. |

|Experience in a community based or human service program. |

| |

|Demonstrated experience in community networking, individual and family support |

|Knowledge and experience that demonstrates empowering people to address matters affecting their daily lives. |

|Wide range of communication and administration skills suited to the Human Services and Community sector |

|Ability to relate to people from a range of cultural and language backgrounds |

|Commitment to attend team and staff meetings and training |

|Current C class driver’s license and certification to work with vulnerable people |

| |

|Tertiary qualification in Social Services, Human Services, or equivalent relevant experience |

| |

|Demonstrated working relationship with government and non government agencies and departments |

|Previous experience working within a quality framework. |

|What you will do in this role… |

| |

|Case Management |

|Complete to a high standard case management support with a minimum of 12 families for a full time case load, and pro rata for part time staff |

|Responsible for actively engaging children, young people and families in identifying, planning and achieving personal and family goals. |

|High understanding of ethical boundaries |

|Support and contribute to the ongoing positive development of the Case Management and UnitingCare staff teams |

|Attend regular Case Management Team Meetings (weekly) and UnitingCare Staff meetings (monthly) |

|Commit to continuous personal development, professional supervision and peer mentoring |

|Introduce and support families to engage in a wide range of community based services and activities |

|Liaise and network with government and non government agencies and services |

|Identify opportunities and provide recommendations for further family engagement |

|Encourage families to contribute their skills and talents in the local community |

|Administration, Information and reporting |

|Report statistical and performance data in an accurate and timely manner |

|Keep accurate records and work within a Quality Management System |

|Maintain and secure records and files in accordance with office procedures and the UnitingCare Kippax Privacy Policy. |
