Independent Volunteer/Coach Requirements??Thank you for choosing to volunteer at Lower Dauphin. Below are the requirements you must complete before you are permitted to volunteer. Once you satisfactorily complete these requirements, you will be permitted to volunteer throughout the District and receive an approved Volunteer badge. This badge will also have an expiration date, which will indicate the date you will be required to present new clearances to the District Office in order to continue volunteering.?Volunteer Information FormPrint and complete the?Volunteer Information Form?to hand in with your other required clearances. This form can also be used as a checklist for all of the requirements and will assist us in getting your approved Volunteer badge.ACT 34 - State Police Criminal History RecordVolunteer clearances are valid for 60 months from date of issuanceGo to the?Pennsylvania Access to Criminal History (PATCH) Website?and select "Submit a New Record Check"Select “Accept” and then complete the required fields with your personal informationNote:? Reason for Request = VolunteerPrint the final PA Criminal History RecordNote:? You will have a copy e-mailed to your e-mail address if you completed that field within the personal informationACT 114 - Federal Criminal History Background Check OR?AffidavitIf you have lived in Pennsylvania for 10 years or more, please complete this?Affidavit?in lieu?of the Federal Criminal History Background Check in accordance with Act 153If you have NOT lived in Pennsylvania for 10 years of more, please follow the instructions below:Volunteer clearances are valid for 5 years from date of issuanceTo complete your pre-enrollment registration telephonically call (844) 321-2101 Monday through Friday, 8:00 AM - 6:00 PM ESTTo complete your pre-enrollment registration online go to? PDE-Volunteer Service Code:?1KG6Y3Choose “Schedule or Manage Appointment”Check “Yes” and then select “Next”Enter personal info required and then select “Next”You will be asked to create an Answer Identifier QuestionEnter the answer and then select “Next”Answer citizenship questions and then select “Next”Answer personal questions required and then select “Next”Enter personal info required and then select “Next”Enter your address info and then select “Next”Choose, from a drop down box, which identification document you would like to bring along for the fingerprinting (i.e. license, passport, etc.)Check “Yes” if the name on the document matches your current name and then select “Next”Enter your zipcode and search for the closet fingerprint locationChoose LocationChoose a date and time, from the drop boxes. Or check “Walk-in” and then select “Submit”Please note that all scheduled appointments take precedence over “walk-ins”You will then see a message that says, “ You have successfully Pre-Enrolled.” You will also receive a follow up e-mail stating the same thing.ACT 151 - Pennsylvania Child Abuse History ClearanceYour Keystone ID is a username that you create and must have 6-10 charactersNote:? You will also receive an official copy sent to the address you provided.Go to the?Pennsylvania Child Welfare PortalSelect the "Create Individual Account", then select "Next", complete required information and then select "Finish"You will be e-mailed your temporary password to the e-mail account you provided in the required informationYou will need to close the window that you created your Child Welfare Portal account, and re-open a new window to proceed with your Keystone ID (username) and your temporary password (sent to you via e-mail) by logging back into the?Pennsylvania Child Welfare Portal?and selecting "Log In"Select “Access My Clearances” and then select “Continue” at the bottom of the pageEnter your Keystone ID (username) and your temporary password (sent to you via e-mail)You will then need to change your temporary password to a password you select, then select “Submit”You will need to log out again, and then go back into the?Pennsylvania Child Welfare Portal?with your Keystone ID (username) and new password you just createdSelect "Log In" and enter your Keystone ID (username) and your password you just created, Select “I have read…” and then select “Next”Select “Continue” at the bottom of the page and then select “Create Clearance Application”, Select “Begin” and then select “Volunteer”Continue completing Part 1 and Part 2 of the Clearance Application, after you submit all the required information you will be able to print your completed PA Child Abuse History ClearanceVolunteer clearances are valid for 60 months from date of issuance.TB TEST REQUIREDNote:? If you have previously submitted these results, you do NOT need to submit them again since they never expire.Must be completed within the last 90 days (when submitting your other clearances) in order to be valid.UPON COMPLETIONAfter you have gathered all necessary required documents, please bring copies of each document to the District Administrative Center [291 East Main Street, Hummelstown] or a school office.Note:? We recommend bringing all of your documents to your scheduled fingerprinting appointment.?Your documents will be reviewed by Human Resources and once they are approved, you will be contacted via e-mail about where you can pick up your approved Volunteer badge.?Questions?
