RDAC is a leading provider of culturally appropriate , comprehensive primary health care and related social and family support services to Aboriginal people and communities across Gippsland. RDAC’s services are provided at the following locations: Sale (3), Morwell, Drouin, Loch Sport, Rosedale and Stratford. RDAC’s services include; General Practice, Health Assessment and Early Detection Screenings, Health promotion and Education, Dental, Audiology, Chronic Disease Management and Care Coordination , Drug and Alcohol counselling, Family, Child and Maternal Health, Home and Community Care, Immunisation and Vaccinations, Social and Emotional Wellbeing services, Allied Health, Client Transport and Corporate Services.Team InformationThe Senior Health Prevention Project Officer will be responsible for the development and implementation of initiatives to strengthen prevention and early detection health practices across all RDAC clinics.Position PurposeThe purpose of this role is to: coordinate, plan and manage the RDAC Working Together for Health initiative through strengthening and expanding preventative and early intervention health practices across all RDAC Clinics.A primary focus will be the implementation of best practice and evidence based initiatives to ensure Aboriginal people have access to early intervention and primary prevention activities with emphasis on:annual MBS 715 Health Checks and MBS income generation associated care planning routine cancer (breast, bowel and cervical) and sexual health screening and testing oral healthtobacco cessation This role will work closely with health centre staff across all sites to identify and support processes, pathways and systems to enable early intervention and primary prevention activities to occur. To achieve this focus, identifying barriers that may impact upon access to RDAC clinics, and identifying individuals who have not had recent health checks or are not being managed through current care plans will be required. ?The establishment of links and referral pathways to external health services to encourage and support an increased use of other health services within the area/region is also necessary. Additionally, this position will undertake secretariat responsibility for the Working Together for Health initiative within the Gippsland region.Skills, Knowledge and CapabilitiesTertiary qualification in a related field of health such as Public Health, Primary Health or Health Administration.Knowledge and understanding of health issues affecting Aboriginal Australians, including the social determinants of health.Demonstrated understanding and commitment to Victorian Aboriginal health, Aboriginal culture, and the philosophy and practice of Aboriginal Community Control.Demonstrated ability to work with Aboriginal organisations, communities, mainstream health organisations and individuals in culturally appropriate ways.Excellent interpersonal and communication skills and the ability to liaise effectively with a wide range of people, work in a team environment and develop and maintain effective working relationships at all levels.Self-motivated with ability to work independently and as an effective team member.A flexible approach, with strong organisational and planning skills, including the ability to effectively manage time and workload, prioritise tasks, and meet changing circumstances, competing demands, interruptions and tight deadlinesAdditional FactorsPossession of a current ‘C’ Class Driver’s Licence (Victoria) is required.Must be able to obtain a Working with Children Card.Satisfactory Police Check.Position FunctionsKey areas of accountabilitiesTogether with internal and external stakeholders develop initiatives to strengthen and expand prevention and early intervention health practices in ACCHOs across Gippsland that encompasses project and evaluation plans.Support other stakeholders to implement evidence-based prevention and early intervention initiatives and where appropriate, innovative preventive health programs through the development of a regional project engagement strategy.Secretariat responsibility for the Working Together for Health initiative within the Gippsland region will include the preparation of a monthly report on behalf of DjillayNgalu and the RDAC for presentation at the monthly Project Control Group meetingsProvide expert advice, recommendations and innovative cost-effective solutions to improve prevention and early intervention-oriented primary health care in all RDAC clinics. Actively work to build prevention into an integral part of core service delivery in all RDAC clinics, including encouraging Aboriginal people and families to have 715 Health Checks.Evaluate and report on the strengths and weaknesses of current prevention system approaches as well as developing and implementing strategic improvement opportunities that includes opportunities for income generationAnalyse relevant health statistics to identify the inhibitors influencing Aboriginal people in accessing 715 Health Checks. Review and consider current workforce capacity, development and change management approaches in order to implement and embed the project initiatives. Participate in the WTfH Statewide Sub-committee inclusive of other ACCHOs and VACCHOs 10. Contribute to the 90 day reporting cycle to VACCHO and act as a portal for distributing VACCHO's WTFH dashboards within the Gippsland region for Ramahyuck, Sale, Morwell and Drouin11.Undertake other duties which are appropriate to the level of the position, as directed by the Regional Practice Manager to meet project objectives.Other responsibility areas (All staff)Quality and Risk ManagementActively participate and incorporate continuous quality improvement and sound risk management principles to all aspects of the role and in accordance with RDAC policiesProfessional Development & EducationActively participate in relevant continuing professional development as required.
