Covid-19 Screening

Question: What code will be assigned for COVID-19 screening once the federal Public Health Emergency (PHE) for COVID-19 expires?

Answer: The federal Public Health Emergency (PHE) for COVID-19 expired on May 11, 2023. Based on the ICD-10-CM Official Guidelines for Coding and Reporting, continue to assign code Z20.822, Contact with and (suspected) exposure to COVID-19, for COVID-19 screening that is performed after May 11, 2023.

Note: This advice is consistent with current coding guidance.

Effective October 1, 2023, the ICD-10-CM Official Guidelines for Coding and Reporting for encounters for COVID-19 screening will be revised. For encounters for COVID-19 screening on or after October 1, 2023, assign code Z11.52, Encounter for screening for COVID-19.

Question: Once the federal Public Health Emergency (PHE) for COVID-19 expires, what code should be assigned for COVID-19 screening that is performed during preoperative testing?

Answer: For an encounter for COVID-19 screening that is performed as part of preoperative testing after May 11, 2023, continue to assign code Z01.812, Encounter for preprocedural laboratory examination, as the first-listed diagnosis and code Z20.822, Contact with and (suspected) exposure to COVID-19, as an additional diagnosis.

Note: This advice is consistent with current coding guidance.

Effective October 1, 2023, for an encounter for COVID-19 screening that is performed as part of preoperative testing, assign code Z01.812, Encounter for preprocedural laboratory examination, as the first-listed diagnosis and code Z11.52, Encounter for screening for COVID-19, as an additional diagnosis.

Question: A 77-year-old patient diagnosed with biochemically recurrent prostate cancer was seen for a hematology/oncology visit to discuss systemic therapies, such as androgen deprivation therapy (ADT). He had previously undergone prostatectomy and salvage radiation therapy for advanced prostate adenocarcinoma. When a patient is diagnosed with biochemically recurrent prostate cancer, status post prostatectomy, would a code be assigned for recurrent prostate cancer or the rising prostate-specific antigen (PSA) levels? How would this case be coded?

Answer: Assign code C79.9, Secondary malignant neoplasm of unspecified site. Also assign codes R97.21, Rising PSA following treatment for malignant neoplasm of prostate, Z90.79, Acquired absence of other genital organ(s), and Z85.46, Personal history of malignant neoplasm of prostate, as additional diagnoses. While R97.21 is a symptom code, it is appropriate in this case since it provides important clinical information regarding the patient's rising PSA.

Biochemically recurrent prostate cancer is characterized by rising PSA levels in spite of local treatment and prostatectomy. The increasing PSA level is an indicator of disease progression (i.e., local recurrence or
