They will be accepting applications forTourist Passports and Consular Report of Birth Abroad (CRBA) between 1000 and 1300 on a Walk- in-basis.Arrive before 1300 with the appropriate outreach checklist and all required items (completed form, passport photo, postal money order, original and copies of supporting documents, return envelope, etc.)Renew by mail: If you have a tourist or government passport issued after the age of 16 and within 15 years, see the Embassy website or outreach checklist for mail-in information.Checklists can be downloaded from http:// us-embassy-outreach-event or picked-up at 374 FSS MPF, Bldg. 316, Room 121.Federal Benefits: Room 207. (Source: 374 MPF FSS)Getting Ready for Tax Season: Whether you do your own taxes or rely on an outside preparer now would be a good time to start thinking about all the tax forms you will need, and to make sure your mailing and e-mail addresses are up-to-date. Following is a list of the most common IRS tax forms:526119013396Form W-2 (the "Wage and Tax Statement") is the IRS tax form that reports wages paid to employees andthe taxes withheld from them.Form W-2G – Is used to report certain gambling winnings (Bingo, slot machines)Form 1099-R is usedfor reporting on distributionsfrom pensions, annuities, retirement. or profit sharing plans, IRAs, and Insurance Contracts. Some of the items included on the form are the gross distribution, the amount of the distribution that is taxable, the amount withheld for tax purposes, and a code that represents the type of distribution made to plan holder.Form 1099-MISC is used to report miscellaneous income. One notable use of Form 1099-MISC is to report amounts paid by a business to a non-corporate US resident independent contractor for services (in IRS terminology, such payments are non-employee compensation). Other uses of Form 1099-MISC include rental income, and royalties.Form 1099-DIV is a form sent to investors who have received distributions from any type of investment during a calendar year. Investors can receive multiple 1099-DIVs. Each Form 1099-DIV should be reported on an investor’s tax return.Form 1099-INT is used to report interest income. The1 of 3form is issued by all payers of interest income to investors at year end and includes a breakdown of all types of interest income and related expenses. Payers must issue Form 1099-INTs for any party to whom they paid at least $10 of interest during the year.Form SSA-1099 is used to report any Social Security benefits that you may have collected during the year. To enter, go to: If you elected to have taxes withheld, it will also show this amount. It is uncommon to have withholdings from social security benefits.Form 1042-S and SSA Form 1042-S is a IRS tax form dealing with payments to foreign persons, including non-resident aliens, foreign partnerships, foreign corporations, foreign estates, and foreign trusts. Both DFAS and Social Security are issuing the 1042-S to some widows and spouses who are receiving SBP and/or social security benefits.You may receive other documents that list income that may have to be reported to the IRS on your tax return. (Source: )DFAS Telephone Self Service:1947475549581Telephone Self-Service is an automated option that makes it easy for military retirees and annuitants to have their 1099-R mailed to the mailing address DFAS has on record.This option can be used 24 hours a day, 7 days a week. There is no need to speak with a representative, wait on hold, or even use a computer.Use the following instructions to get your 1099-R through Telephone Self-Service:1. Call 800-321-1080.Select option 1 for Self-Serve.Select option “1” if you are a Retiree or option “2” if you are an annuitantEnter your Social Security Number when prompted.Your 1099R should be in the mail within 7-10 business days to the address we have on record.Is your address no longer current? Learn about changing your address here.(Source: dfas.mil)Federal Benefits Open Season Dental and Vision:507440614377Eligible TRICAREbeneficiaries may purchase dental and vision coverage through FEDVIP duringthe Federal Benefits Open Season. If you’re already enrolled in a FEDVIP dental or vision plan and you don’t want to make a change, your enrollment will continue in 2020. If you wish to make changes to your existing plan, you must do so during open season.You may choose between multiple vision and dental plans and carriers. Compare 2020 plans and rates using the FEDVIP plan comparison tool. Open Season runs from 11 November to 9 December 2019 for 2020 coverage. All military retirees regardless of branch of service and their spouses are invited to attend.Volunteers Welcome!!! Volunteering is a rewarding experience. There are many programs and activities within the military community that could not exist if it were not for the many volunteers doing the work to make things happen.Be a Volunteer at the Yokota RAO. if you have been looking for a rewarding way to stay involved in the Yokota Military Retiree community, then volunteering may be the answer for you. Here at the Yokota Retiree Activities Office, you can easily join our staff of volunteers.The RAO is here to assist the retiree community (retirees, their family members, and surviving spouses), regardless of the branch of service.
