JOB DESCRIPTION

REPORTS TO: Senior Social Worker

SALARY: Band 6 £26,302 - £35,225 pro rata

HOURS: 20 hours per week

LOCATION: Newry & Dungannon

The Social Worker will work as part of a multidisciplinary team providing an understanding of the psychosocial and emotional needs and aspects of care for patients and families in relation to illness, loss and bereavement. The Social Worker will be responsible for the provision of social work services to patients and families.

ESSENTIAL CRITERIA

To hold a professional Social Work qualification e.g. MSW, Diploma/Degree in SW, CQSW or CSS.

Have successfully completed an AYE.

Registered on the NISCC Register / Social Work Section.

Have three years post qualifying experience in a paid social work post with two years at Band 6.

DUTIES AND RESPONSIBILITIES

• To work as part of a multi-disciplinary team providing a flexible social work service to patients, carers and significant others.

• To participate in multi-disciplinary ward meetings.

• To take referrals from doctors, nurses, patients and their families and from agencies that have known the patient or family outside the Hospice in situations where psychological, emotional and social problems occur.

• To make an assessment of the patients and families' circumstances and resources and together with them, will formulate care plans in conjunction with the multi-disciplinary team.

• To undertake direct work with patients and families to enable them to adjust to changing situations, using individual, group or family therapy involving other members of the multi-disciplinary team as appropriate.

• To liaise with Community Trusts, Social Service Departments, Primary Health Care Staff and other appropriate organisations, negotiating care packages, Nursing Home placements etc, based on the patients' and carers' assessed needs.

• To provide advocacy and welfare rights information relating to Statutory Social Security Benefits etc

• To undertake regular professional supervision to support and enhance professional practice and personal development and to maintain registration with NISCC.

• To participate in Bereavement Support Services, co-ordinating and organising Bereavement meetings for adults and children.

• To keep updated records of all Social Work involvement which will facilitate communication within the Hospice, with other professionals and agencies involved with patients and their carers.

• To participate in the multi-disciplinary teaching programme of the Hospice and external training events as required.

• To engage in practice based research and keep statistics as and when required.

• To support the Senior Social Worker in the continued development of the Social Work Department in line with the ethos of the Hospice Movement.

• To keep informed of developments and research within social work.

• To carry out responsibilities with an awareness of ethical, statutory and legal requirements and implications.
