|Under direction of the Department of State Hospitals (DSH), Patient Management Unit, Nurse Consultant, Program Review, the Health Program Specialist I(HSPI) |

|performs the more responsible, varied, and complex technical analytical staff services assignments. The HSPI is responsible for the development, coordination and |

|monitoring of all patient movement within DSH. |

| |Function as a lead and highly skilled program consultant responsible for oversight of the patient referral process to ensure compliance with |

|30% |existing policies. Reviews, researches, analyzes, and makes recommendations regarding the most complex patient referrals in the unit. Manages, |

| |monitors, and analyzes the centralized waitlist to ensure adherence to timelines, waitlist balancing, and making appropriate patient placements.|

| |Identify, analyze, and make recommendations and reports on outstanding issues and challenges; identify areas of non-compliance; recommend |

| |corrective action plans to management; and monitor implementation of corrective action plans to ensure timely completion. Present research |

| |As lead Subject Matter Expert (SME), formulate processes and procedures related to the most complex issues related to patient placements and |

|25% |coordination of all patient populations. Develops screening criteria for appropriate patient placement. Gathers and disseminates information on |

| |promising strategies, program successes and models. Maintains expertise in the knowledge, policies, procedures, laws, and regulations governing|

| |patient movement. Performs gap analysis, identify challenges, and make recommendations to management based on existing policies, procedures, |

| |regulations, laws, and MOUs. Lead expert for elevating serious and time sensitive patient issues to senior management. Develops written |

| |As lead on the most complex referrals and issues, serves as liaison with internal and external stakeholders. Provides oversight and |

|5% |coordination with the hospitals’ liaisons. Coordinates and facilitates interdisciplinary workgroup meetings by creating agendas, meeting |

| |materials, and producing minutes. Provide consultation and guidance to Departmental facilities and stakeholders regarding the varying processes|

| |for all commitment types. Create and oversee implementation plans for new policies. Provides training to staff and/or stakeholders. |

| |As lead, coordinates the activities assigned to the Department related to various aspects of the patient movement process which includes, but is|

| |not limited to, scheduling and facilitating meetings, assisting with development of process reviews of admissions processes to identify |

| |opportunities for capturing efficiencies, coordinating patient screening, placement, admissions and discharges, assisting with patient |

| |resolution, and patient documentation sharing for CDCR inmates-patients, county commitments, and DSH patients. Responsible for maintaining |

| |patient movement tracking systems and/or databases. |
