Dear {14C Employer} Employee and Parent/Guardian:

I would like to take this opportunity to introduce our agency, the State of Hawaii, Department of Human Services, Division of Vocational Rehabilitation (DVR). DVR is a state-federal program for eligible individuals with disabilities who require assistance to prepare for, retain, regain, or advance in competitive, integrated employment.

President Obama signed a new law called the Workforce Innovation and Opportunity Act (WIOA) on July 22, 2014. The changes in this new law try to make sure that people who want to work in the community for at least minimum wage, know about the programs that can help them achieve this goal.

You are being given this information because you work for {14C Employer}. If you would like to continue working at less than minimum wage, the new law requires you to receive training on the various work options available to you.

To make it easier for you, DVR has made plans to hold small group meetings at {14C Employer}. Guardians, or people who are important to you, are invited to participate in the training with you. The training will take about an hour. After you finish the training, you will be given a form that says you completed it. It is required by law that this training occur twice a year for the first year, and every year after that, for as long as you continue to work for {14C Employer} and are being paid less than minimum wage. You will be notified when this training happens again.

What now?

Attached to this document are two forms: A refusal form and a verification form. If you want to keep working at {14C Employer} for less than minimum wage, the verification form should be filled out and returned to {14C Employer}.

Here are your two choices:

Option 1: If you choose to go to the training, please fill out the attached ‘Verification Form’ and return it to {14C Employer}.

Option 2: You do have the right to not participate in the training. If you make this choice, you will need to meet with {14C Employer} and your team to discuss how this may impact your job. If you choose not to participate in the training, please fill out the attached ‘Refusal Form’ and return it to {14C Employer}.

It is important that you make an informed choice on where you work. If you want to change your work situation, DVR can help talk to you about your situation and how a change in work may impact you. While WIOA requires these services to be provided by DVR, the larger purpose is the introduction to the possibility of competitive, integrated employment with DVR’s assistance.
