Please call this number for questions concerning scheduling of frozen section and sentinel node cases, handling of specimens (including unusual specimens), this procedure, needed supplies, or final report.

GENERAL SPECIMEN INFORMATION:

1. Gross specimen examination is done by a pathologist or pathology assistant under the supervision of a pathologist between 9:00 AM and 5:00 PM, Monday through Friday. Tissue received after 4:00 PM will generally not be examined and submitted for processing the same day due to inadequate fixation. The department is closed on weekends and holidays.

2. A pathologist is always available for intra-operative consultation (see Intra-operative consultation or Frozen section instructions).

3. Microscopic slides are given to the pathologist for examination as soon as they are available after processing. Reports for routine cases are generally completed within 24 hours after receipt in the department. Complex or large specimens may require more time.

4. Completed reports are available in Cerner. Hard copies of reports are either placed in physician’s hospital mailboxes or sent to their offices via fax or mail.

5. Cases designated ‘rush’ or ‘stat’ by a physician should be hand delivered to Surgical Pathology prior to 4:00 PM to ensure next day results.

6. For specimens submitted for specialized studies, please refer to the separate instructions or call the Pathology department.

7. Specimens received without adequate information or with insufficient, questionable or misleading labeling cannot be examined and processed until the deficiency is corrected. The department will make every effort to have the deficiency corrected as quickly as possible but it may occasionally cause delay in reporting.

GENERAL SPECIMEN REQUIREMENTS:

1. Complete the APR Hospital Histology/Cytopathology form. In the form, please complete appropriate areas with particular attention to the following:

a) Provide the PATIENT’S FULL NAME, BIRTHDATE AND SEX in the upper left field.

b) CLIENT INFORMATION box – list doctor doing procedure first, then provide names of all doctors to whom copies of the report are to be distributed.

c) COLLECTION DATE and TIME.

d) ICD10 CODES – please provide ICD10 code(s) or select code(s) from the list.

e) SURGICAL PATHOLOGY field – list all tissue specimens submitted – be as specific as possible.

f) DIAGNOSIS/CLINICAL INFORMATION – the more information, the better.

g) BILLING INFORMATION – this may be entered on the form or a separate print-out.

2. Place the specimen into container of 10% neutral buffered formalin. Try to use a minimum of 5:1 volume of formalin to volume of tissue; 10:1 or greater is optimal. Be certain that container is securely closed. Do not let formalin contact skin or mucous membranes – please refer to Safety Data Sheet (SDS) information.

NOTE: Attach a 10% formalin warning sticker if not already on container.

3. Label the specimen container with both the patient’s name and specimen source.

4. Place the paperwork (requisition form, billing information) and specimen container into a plastic zip lock bag for transporting.

5. Place bag with paperwork and specimen container into designated lab pick-up area.
