Job Title: Community Service Provider

Department: Mental Health Support Services

Reports To: Community Services Supervisor or Mental Health Supervisor

FLSA Status: Non-Exempt

Summary Provide mental health services to individuals with mental illness under the guidelines of the Woodview Support Services Community Service Program in the following settings: a consumer's home, the home of a consumer’s relative or significant other, a consumer's job site, the community such as psychosocial clubhouse, drop-in center, social setting, classroom or other place in the community.

Essential Duties and Responsibilities include the following. Other duties may be assigned.

Implement services in the treatment plan for each consumer under the direction of the Woodview Support Services Community Service Program. The plan may include services that instruct, assist and support a consumer in areas such as interpersonal communications, community resource utilization and integration, crisis assistance, relapse prevention, budgeting, shopping and healthy lifestyle skills and practices, cooking and nutrition, transportation, medication education, mental illness symptom management, household management, employment related skills and transitioning to community living.

Establish therapeutic relationships with assigned consumers and meet with each consumer as needed according to contracted hours.

Document all services provided accurately, completely and within required deadlines

Must submit all documentation in a timely manner according to the determined guidelines and deadlines

Complete initial and ongoing training requirements defined for the position at required minimum standards

Meet with the Mental Health Professional Clinical Supervisor at least monthly for training and treatment plan review

Must attend and complete all required training necessary for the position within the appropriate time frames

Report problems, concerns, client grievances to your supervisor immediately.

Qualifications

The CSP Worker must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. CSP Workers must meet the following requirements:

- Must be at least 19 years of age

- Must hold a high school diploma or equivalent

- Within the previous 10 years, must have at least 1 year of experience as a primary caregiver to an adult with a major mental illness or TBI

Language Skills

Ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write simple correspondence. Ability to effectively present information in one-on-one and small group situations to consumers, and other employees of the organization.

Mathematical Skills

Ability to add, subtract, multiply, and divide in all units of measure including time, using whole numbers, common fractions, and decimals.

Reasoning Ability

Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.

Other Skills and Abilities

Ability to establish and maintain positive relationships with consumers and staff

Other Requirements:

Must be able to pass a Department of Human Services background study check

Must hold a valid driver's license and pass current company insurance company's guidelines for driving.
