CompetencyTopic: Mock Code Blue—First 5 MinutesObjective: To demonstrate the first 5 minutes of caring for a patient in cardiac arrestMethod of Evaluation Key:O = ObservationRD = Return DemonstrationT = TestV = VerbalCompetency Criteria Method of EvaluationMetNot MetKnowledge:High-quality CPR components for BLS providers 1. Recognition of cardiac arrest. 2. Activation of emergency response system 3. Compression-ventilation ratio without advanced airway 4. Compression-ventilation ratio with advanced airwayPriority based team resuscitation roles: 1. Compressor: Assess patient; does five cycles of chest compressions 2. Monitor/Defibrillator: Applies pads to patient; operates the monitor/defibrillator 3. Airway: opens the airway; provides bag-mask ventilation at 30:2 4. IV/IO Medications: Initiates IV/IO access; administers medicationsAdult cardiac arrest algorithm for ventricular fibrillation/pulseless ventricular tachycardia and asystole/pulseless electrical activity.Adult bradycardia with a pulse algorithmAdult tachycardia with a pulse algorithmImmediate post-cardiac arrest care algorithmSkills:Assesses for unresponsiveness and/or an absence of breathing or presence of abnormal breath sounds for 5-10 secondsActivates EMS... Shout for help; call code blue; dial 18. Get the crash cartPerforms pulse check for 5-10 secondsIf pulse is present, begins rescue breathing, one breath every five to six secondsIf pulse is absent, initiates chest compression rate of 100-120 per minute Monitor/defibrillator: turn machine on and place defibrillator pads on patient. . Assess rhythm. If rhythm is shockable, charge defibrillator and deliver shock. Continue CPR for 2 minutes and after 2 minutes, reassess rhythm for shockable rhythm. Airway: Open airway with head tilt-chin lift. BVM assembled. Use C-E technique for BVM ventilation. Avoid excessive ventilation. Two breaths after 30 compressions, delivered over 1 second causing chest to rise. IV/IO Medications: IV access. Medication cartridge assembling. Administration of ACLS medications and flush. ROSC: maintains oxygen saturation > 94%; treats hypotension of SBP < 90 mmHg; 12-lead ECG; transfer to critical care. Attitude:Maintains patient and rescuer safetyIncreases patient’s chance of survival by obtaining AED and defibrillating a shockable rhythm as early as possible.Advocates for the provision of high quality CPR to increase the chance of survivalComments: References:Policy #11945: Code Blue/White - Response & InterventionsAmerican Heart AssociationForms of evidence that may be submitted to demonstrate competency:Skills Validation Fair Employee’s SignatureEmployee NumberDate Validator’s SignatureEmployee NumberDate ................
