How to Request a Title I Campus Contact Stipend

Complete the form, attach a copy of the record of teaching certificate and email it

to your principal.

The principal will sign the form, attach a copy

of the employee's record of teaching certificate and email it to the Title I Specialist for their campus.

The Title I Specialist will review the form, verify the employee is eligible to receive the stipend

and forward for processing.

Step 1

Employee completes the Request for Campus Contact Stipend Form. A copy of the record of teaching certificate must also be attached. The stipend amount is preset at $2,400 per year. The form is to be signed electronically by the employee requesting the stipend to be paid. The employee will complete the Campus Name, Campus No., Employee Name, Employee ID, Job Title, Date Effective and the Campus (Organization) Number in the budget string. Attach a copy of the record of teaching certificate for the principal to forward with the stipend request form. The TEA Certificate Lookup page can be found at .

Finally, the employee will digitally sign the form (see screenshots below for instructions), save a copy, and then email the form and a copy of their record of teaching certificate to his/her principal.

Step 2

Principal reviews, signs and emails the form and the copy of the record of teaching certificate to the appropriate Title I Specialist in External Funding. The employee's principal will review the form. If she/he approves the request, the principal will digitally sign the form, attach a copy of the record of teaching certificate for the employee and email them to the appropriate Title I Specialist for their campus. Note: If the principal is not offering a stipend, please check Designee on the stipend request form and the campus budget will not be charged for the stipend.

Step 3

The Title I Specialist reviews, verifies the form and forwards the employee's information to the appropriate person for processing.

NOTE: The request will not be accepted without the employee and principal electronic signatures.

How to Digitally Sign This Form

If you already have a Digital ID, select My existing digital ID in the Add Digital ID window.

The Sign Document window will appear. Enter your password then click Sign.

If this is your first time creating a Digital ID, The Add Digital ID window will appear.

Select A new digital ID I want to create now. Click Next.

Select New PKCS#12 digital ID file. Click Next.

Complete these fields: Name Organizational Unit (campus name) Organization Name (HISD) Email Address

Click Finish.

Select Browse. The Save Digital ID file window will appear. Choose a file location and Save. Enter a password in the Add digital ID window then select Finish.

In the Sign Document window enter your password then click Sign.

How to Request a Campus Contact Stipend

The Save As window will appear for you to save the signed document.

Your signature will appear on the document. Email the document to the appropriate person as instructed above.
