Your AvMed Entrust Plan offers comprehensive health coverage at prices you can afford and features:

AvMed values the unique role of the patient-physician relationship. Your Primary Care Physician (PCP) plays an integral part in coordinating your care with your entire team of specialists and other healthcare providers. Your PCP also manages authorization requirements, when appropriate. Because of your PCP's unique vantage point, we strongly encourage you to leverage him/her as your personal medical historian. When you ask each AvMed Participating Specialist you visit to forward all associated records to your PCP, he/she has the ability to consider each detail of your complete care history when guiding you in any decision. Having all outside records in one location also gives you peace of mind by minimizing your personal burden related to maintaining, updating or even remembering each detail of your care. This simple step keeps your focus where it should be-on your health and the care choices you and your PCP will make together.

In your AvMed Entrust Plan, it also sets the stage for that strong relationship's position in the center of your healthcare universe. That's the foundation of AvMed CenteredCare, a core feature of your Entrust Plan. To ensure full coordination of your care, AvMed requires you to select a PCP from this Directory and to work with him/her anytime you need a referral to an AvMed Participating Specialist.

AvMed strongly believes that preventive health is the cornerstone to staying healthy. That's why your AvMed Entrust Plan provides preventive care services at no cost, even before you've met your annual deductible. AvMed truly wants our Members to take active roles in managing their health and wellness. We encourage you to get the proper age and gender specific screenings and immunizations to keep you healthy and prevent future health problems.

II. ACCESSING CARE

In an effort to keep our Members informed, this section contains general information about accessing care with your AvMed Entrust Plan. For your plan's specific benefit information, please refer to your Contract or Summary of Benefits and Coverage. Should you have any questions, please contact our Member Engagement Centerat the number listed on your AvMed ID Card.

Your Primary Care Physician (PCP) Your PCP provides routine and preventive care while assisting you in making important medical decisions. Your PCP can perform physicals, see you for most of your healthcare needs and help coordinate your care if you need to see specialists or access behavioral services. Your PCP should know your medical history and can be a valuable resource for information and treatment.

Each covered member of your family may choose the same or different PCPs. You can find a list of participating doctors in this Provider Directory or on AvMed's website at . To select or change your designated PCP, please contact our Member Engagement Center at the number listed on your AvMed ID Card.

Visits to Specialist Providers Your Primary Care Physician (PCP) knows your medical history and is best qualified to determine when your care requires specialist involvement. Please consult your PCP for advice.

Emergency Care When possible, always discuss your care options with your Primary Care Physician (PCP) if you need immediate medical attention. Because you never know when that may occur, be sure to gather information on your next visit including after-hours care, weekend accessibility and any alternate phone numbers you should know. Knowing your options in advance will help if your PCP isn't available when your accident or injury requires immediate attention. In those cases, proper planning can save you time, money and stress.

AvMed Participating Providers AvMed is committed to quality healthcare and partners with a broad network of providers, committed to keeping you healthy. AvMed contracts with doctors in private practice who agree to see AvMed Members within certain time frames, depending upon the Member's condition. AvMed Participating Providers also agree to certain standards of care for our Members in regards to wait times and accessibility. AvMed Participating Hospitals, Facilities & Allied Services AvMed Members have access to one of the most versatile facility networks in the state, made up of hospitals, skilled nursing facilities, diagnostic centers, laboratories, ambulatory surgical centers, home health, urgent care centers, pharmacies, vision companies, durable medical equipment providers and much, much more. To be an AvMed Participating Provider, healthcare facilities must meet rigorous credentialing standards based on quality. AvMed Quality of Care Standards leverage those of nationally recognized professional organizations and are regularly monitored for provider adherence. AvMed supports our providers in their efforts to meet or exceed quality standards.

Board Certification AvMed considers Board Certification a significant credential in evaluating providers. Our Board Certified Network Providers have completed advanced training in an approved hospital residency and/or fellowship program. Each specialty board establishes its own requirements for providers to achieve Board Certification and, within this Directory, we've identified this subset with a star ( ).
