Instructions for Preparation of Lineage Papers

1. Please do not submit your application for membership until you have received an invitation to join. The Membership Inquiry form, posted on the website, is available for inquires.

2. The application must be typed and completed in the space provided. Please do not attach documents to the application. Only one copy of the application and one set of documentation are required.

3. Names should be written in full, including maiden names of wives, if known. State the male first in each generation. The line of descent may be emphasized by capitalizing the last name of that ancestor in each generation.

4. Include dates of birth, marriage and death in the following format: 4 Oct 1633.

5. If information is unknown or not applicable, leave the space empty.

6. The proof of each generation must be listed on the lineage paper by document and volume (title and applicable page numbers). Documents providing proof of each generation/event are required. Pertinent information should be underlined in red on each proof for easy verification.

7. Verifiable sources may include: birth, death and marriage certificates; federal and state census records from 1850 or later; tombstone transcriptions and photographs (with the name and location of cemetery stated on reverse of photograph); Bible records (with copyright page included), published town histories and vital records; published family genealogies IF sources are cited.

A copy of the title page from all sources must be provided.

Record copies from other lineage societies MAY be acceptable. See last page.

8. The application should be printed front & back on acid-free, letter-sized paper.

Submission of SDVAFF Lineage Papers

1. One copy of the completed application should be mailed with all proofs to the Registrar-Genealogist General at the address below.

2. If the original application is not approved by the Registrar-Genealogist, an alternate pedigree may be submitted within one year.

3. Supplemental applications may be submitted later, using the same form as the original application.

Membership Fees

1. Life Membership + Application Fee = $300.00

If an application is not approved the $200 Lifetime membership fee will be refunded, $100 application and genealogist fee are not refundable. The Lifetime fee includes a certificate.

2. Supplemental applications may be submitted for a fee of $30.00 each. If the application is not approved, $20 will be refunded. The genealogist fee is not refundable.

Certificates for supplementals may be purchased for $10.00.

Prepare two checks as follows:

For initial applications – One check in the amount of $300, payable to “SDVAFF” and one check in the amount of $60, payable to the Genealogist whose name is shown below.

For supplemental applications – One check in the amount of $20, payable to “SDVAFF” and one check in the amount of $10, payable to the Genealogist whose name is shown below.

Mail your application, checks, and proof documents in one envelope by regular mail that does not require a signature for acceptance. DO NOT mail binders or use other forms of binding.

Record Copy Applications

Although this Society accepts record copies from those societies listed below, the Registrar/Genealogist reserves the right to request additional documentation in order to approve any application to this Society.

A Record Copy of an application to one of the listed societies must contain all original source citations. In order to make the application valid as a Record Copy, you must include all applications sufficient to make your collection of source materials known as though they were being reviewed for the first time. References to “see original application” or other similar wording do not accomplish this.
