Welcome to the Carl H. Lindner College of Business at the University of Cincinnati! We are excited that you have chosen a faculty position within our College at the University of Cincinnati. This document contains important new hire information. Please make sure that you complete the steps outlined on the new faculty checklist. The table of contents outlines information included in this document and allows you click on the page number to be directed to the specific page you are seeking. At any point in the document, you can click on the bottom right of the page to return to the checklist or table of contents. Update your contact information, sign up for direct deposit, and update withholding information. See page 7 for instructions. Sign up for benefits (please note the portal will not be open until your start date) You will sign up for benefits online via employee self-service. Please see page 6 for benefits information and page 7 for information on employee self service Set up voicemail and record personal greeting See instructions on page 29. Pick up UC ID badge and keys UC ID can be picked up at 4 Edwards Center on UC West Campus. Keys can be picked up at the same location. Failure to turn these documents in will result in a delay in your first paycheck. Please turn the documents linked below in at HR at University Hall within 3 days of your start date. Please see the front desk attendant at the HR Department to turn in the documents. You do not need an appointment. HR's location and information is below. Benefits Information

You have 45 days from your start date to enroll in benefits. Your benefits will be effective October 1, 2015. You can sign up for your benefits by using employee self-service, please see instructions page 7. Below are important links for reviewing your benefits options. It is recommended that you read the benefits overview first and then click the additional links below for more specific information on each plan. NEW HIRE ENROLLMENT ? ONLINE BENEFITS ENROLLMENT INFORMATION

ESS features a `road map' on each screen which will show which step of the process you have completed.

Many screens will have four steps: Overview, Edit, Review and Save/Confirmation.

ESS Getting Started

? Access the UC Flex Portal at ; the UC Flex portal can be accessed at home or on campus.

? Enter your Central Login Service Username (6 + 2) and Password (also utilized to log into Exchange e-mail, Blackboard etc.). If you have probems logging in contact the UCIT Help Desk at 513-556-4357.

? Click Log on to access the UC Flex Portal.

The EMPLOYEE SELF SERVICE screen provides three choices:

? Benefits and Payment ? use this link to enroll in the university's benefit plans, view benefit particpation, view your pay statement, view/edit W-4 tax withholdings, or view/edit bank information.

? Personal Information ? use this link to view/edit/add address information, bank information, family members/dependents/emergency contact/beneficiaries, personal data, W-4 tax withholdings, and Ethniticy, Race, and US Veterans status. You must add your dependents to your record PRIOR to being able to enroll them in the university's benefit plans.

? Your Pay Statement ? click here to view your pay statement.

Click HELP located in the top right hand corner of the screen to be directed to the UC Flex help website. The help web site will provide step by step directions on how to use Employee Self Service.

Purpose: Enter, change or delete your addresses. If you are a student employee, an address change does not automatically update your address information in the Registrar's Student System.

? Click on the Personal Information link from the overview screen. ? Click on the Address link to view and/or update the address on record. ? After viewing the address on record click Exit located at the bottom of the screen to return

to the overview screen. Click Edit to update or modify the address on record. ? Make the neccesary changes to the address, click Review at the bottom of the screen to

review the changes (this is a required step before saving the changes). Click Previous Step at the bottom of the screen to navigate to the previous screen or Exit at the bottom of the screen if no changes are desired to be made to the address.

? After reviewing the changes, click Save located at the bottom of the screen to save the changes made to the address.

? Click the Go to Employee Self Service hompage link to be directed back to the employee self service overview screen.

NEW HIRE ENROLLMENT ? ONLINE BENEFITS ENROLLMENT INFORMATION

Personal Information Bank Information

Purpose: Enter, change or delete your bank information. You will need your bank routing number and your checking/savings account number to complete the banking information entry.

? Click on the Personal Information link from the overview screen.

? Click on the Bank Information link to view and/or update the bank information on record.

? After viewing the bank information on record click Exit located at the bottom of the screen to return to the overview screen. Click Edit to update or modify the bank information on record.

? Make the neccesary changes to the bank information on record, click Review at the bottom of the screen to review the changes (this is a required step before saving the changes). Click Previous Step at the bottom of the screen to navigate to the previous screen or Exit at the bottom of the screen if no changes are desired to be made to the bank information.

? After reviewing the changes, click Save located at the bottom of the screen to save the changes made to the bank information.

? Click the Go to Employee Self Service hompage link to be directed back to the employee self service overview screen.

Personal Information ADD FAMILY MEMBER &

DEPENDENT INFORMATION

Personal Information Personal Data

Purpose: Family members and dependents must be entered in the system in order to be enrolled for medical or dental coverage or to be named as a beneficiary for your life and personal accident insurance. Adding names and personal data does NOT add your dependent to your health insurance, you must SELECT dependents when enrolling for coverage.

TO ADD dependents (spouse, domestic partner, children) to your personal data: ? Click on PERSONAL INFORMATION (located in the red toolbar). ? Click FAMILY MEMBER/DEPENDENTS/EMERGENCY CONTACT/BENEFICIARY. Click on the appropriate type of addition (spouse, child, etc.). Enter applicable information. When finished, click the REVIEW button at the bottom of the screen. After the information has been reviewed, click the SAVE button at the bottom of the screen. ? Click GO TO FAMILY MEMBERS/DEPENDENTS OVERVIEW if you desire to enter additional family member, dependent, emergency contact, or beneficiary information. Select GO TO EMPLOYEE-SELF SERVICES HOMEPAGE if you have completed all family members, dependent, emergency contact, and beneficiary entries. ? In order to add over age 26 dependent(s) to your medical or dental coverage, you must contact the Human Resources at 513-556-6381. Ask the HR representative about the documentation required in order to add over age 26 dependents.

Purpose: Verify the correctness of your personal data.

? Click on the Personal Information link from the overview screen.

? Click on the Personal Data link to view and/or update the social security and name on record.

? Click Edit to view the date of birth on record.

After reviewing the personal data on record click Exit located at the bottom of the screen. If the name, social security number, or date of birth requires an update or change, contact the Human Resource Service Center at (513) 556-6381; this information cannot be edited using Employee Self Service.
