Register for Portal Access

1. Go to premiumbilling. 2. Click the New User Registration. 3. Enter the Registration Code from the New Member

Login or SPM Welcome letter you received in the mail. 4. Enter your First Name, Last Name and SSN. 5. Review the user license agreement, select the I Accept check box, and then click the Submit button.

6. Review the user license agreement, select the I Accept check box, and then click the Submit button.

7. Set up your username and password. 8. Click Submit & Create Account button. 9. You will be returned to the login home page. Enter

your username and password and then click the Sign In button.

Important:

? Sometimes these fields are already populated with information. Clear each field and manually enter all required information. The auto-populated information can fail to validate correctly which can cause an error or incorrect account setup.

? Select the Remember Me check box if you want your username to be automatically populated the next time you sign into the Member Portal.

? You must enter your password each time you sign into the Member Portal.

Retrieving Login Information

1. On the Sign In window, click the Forgot your username or password link.

2. Enter your Email Address and then click the Submit button. A message box displays to inform you that an email containing your username was sent to you. A member should check their spam folders if they do not receive this email in their inbox within a few minutes. Welcome Page

The Welcome page is the first page you see when you log on to the Member Portal. From this page, you can navigate to all the functionality in COBRA & Direct Billing. You can quickly return to the Welcome Page from any other page in the system by clicking the COBRA & Direct Billing icon in top left corner of the window. ? The information bar across the top of the window contains your name, the date and time of your last

login to the portal, and a link to sign out of the portal. You can click on your name to quickly open the page where you edit your personal information.

? The Profile menu contains links to change your personal information, view your plans, and view your registration information.

? The Payment Info menu contains links to the tools that summarize your past and upcoming payments.

? The Preferences menu contains the links to change your password and see how communications are sent to you.

? The Communication Activity menu links to a listing of all the letters your Administrator has sent to you.

Profile

The Profile menu contains links to change your personal information, view your plans, and view your registration information. To update personal information, do the following: 1. Expand the Profile menu and then click Personal Info.

You can also access the personal information window by clicking your name in the top left corner of the Welcome Page window. 2. Click the pencil icon to the right of your name. 3. Enter the changes to your personal information and then click the Save button.

Viewing Plan Information

You can view your current plan selections through the Member Portal. To view your current plan selections, do the following: 1. Expand the Profile menu and then click Product

Selections. 2. A list displays that contains your current plans. There

is a note under the list that contains the starting date used to calculate the amounts in the list. ? Product - The unique title used to identify the plan. ? Type - The category for the insurance benefits (for example medical, dental, or vision.) You can have multiple plans, but you cannot have two plans of the same type. For example, you can have separate medical, dental, and vision plans but you cannot have two different dental plans.

? Coverage - The combination of individuals covered under the plan (e.g. Member Only, Member + Spouse, Family, etc.)

? Effective Date - The date your coverage under the plan began.

? Amount - The amount of the premium for the coverage period.

? Subsidy - The amount of the premium paid by a third party (typically your employer.)

? Amt. Due - The total amount you owe (the current premium minus any subsidy and plus any outstanding amount).

Viewing Registration Information

You can view your portal registration and login information. To view your registration information, do the following: 1. Expand the Profile menu and then click Registration

Info. 2. Your username, Registration Code, and Registration

Date display for reference.

Payment Info

The Payment Info menu contains links to the tools that summarize your past and upcoming payments. To make a payment, do the following: 1. Click the Make Payment button. This button displays on multiple windows in the Member Portal. The

amount and due date of your next payment display above the button. 2. Select the Payment Method and then click the Next button. You can pay using a debit/credit card (including an HSA (Health Savings Account) card), a checking or savings account, or you can mail a check or money order. 3. If you choose to send a paper payment, the Mail Payment window displays which contains instructions and the remittance address for a paper payment. Click the Print button to print a copy of the paper payment instructions. Click the Close button to return the Member Portal Welcome page.

4. Enter the Payment Amount for a card or bank account payment and then click the Next button. You can pay either the exact amount due for the next premium month or another custom amount you enter. Any

custom amount must be less than the amount of your premium. The system will not process a payment that

is greater than your monthly premium in one transaction. Note: There is a $20.00 convenience fee added to all payments made through the Member Portal. A summary of your total payment amount including this feed is plays on the right of the window.

5. 5. Enter the Payment Information and then click the Next button.

6. A summary of the payment and payment method display at the top of the Review & Submit window and

the Payment User Agreement displays at the bottom of the window. Select the check box to agree to the terms of service and then click the Make Payment button. 7. A confirmation message displays to indicate if the payment was approved or declined. ? Approved Payment - Click the Print button to print a copy of the payment confirmation. A copy of the confirmation is also sent to your email address. Click the Close button to return the Member Portal Welcome page.

? Declined Payment ? A message will display advising payment declined.

Viewing Payment Information

There are tools in the Member Portal that summarize payment information to help you track past and upcoming payments. These tools are accessed by expanding the Payment Info menu. The Payment Schedule tool shows past and upcoming payments in a bar chart format. Each bar represents one premium payment month. The amount paid, owed, and due for each month displays under each bar. A green bar indicates a past payment and a gray bar indicates an upcoming payment. Click the orange arrow that displays to the right of the chart to display additional future months in the chart. Click on a bar to display additional payment information about that month. The additional information displays under the chart. It shows how the payment was applied to your plans and account. In the event of an over payment, the system automatically applies the additional portion to the next premium due.
