Memorandum of Understanding

Between Geta Grant Agency and the San Vente Treatment Teams

This Memorandum of Understanding is entered into for the period of July 1, 20xx through June 30, 20xx by Geta Grant ( a non-profit corporation) and the San Vente Treatment Teams (SVTT), (a non-profit corporation), hereafter SVTT.

Purpose of Agreement

Geta Grant will provide drug testing and outpatient alcohol and drug treatment services to the SVTT employees and clients.

Terms of Agreement

Geta Grant will provide drug testing services Monday through Friday from 1:00 to 5:00 P.M. at 1720 Wayside, Grant City. Drug testing results will be reviewed by Geta Grant Director of Clinical Services and the results will be transmitted to the designated individual of the SVTT program within 7 days of testing. The cost of the drug testing service is $70 per test ($50 for dip-stick or breathalyzer tests). Geta Grant will invoice the SVTT monthly for these services. Payment is due within 30 days of billing.

Geta Grant will provide culturally-sensitive alcohol and drug outpatient treatment services for the SVTT clients who will be referred to the agency for treatment. SVTT will provide appropriate cultural sensitivity training to the staff of Geta Grant. The SVTT will pay for the Addiction Severity Index (ASI) to be completed on each client ($300/client) that screens positive for drug or alcohol use indicated by test results to be more severe than ‘light or occasional use’. SVTT will also provide a co-pay of $10 per group meeting for clients attending client treatment groups (24 meetings/treatment series = $240); SVTT clients attending client treatment groups will pay a co-pay of $2.00. A copy of the ASI will be sent to the designated staff at SVTT with client release. Geta Grant will invoice monthly for these services and payment is due within 30 days of billing.

Employer-Employee Relationship

No employer-employee relationship is implied or intended in this agreement, especially with respect to insurances or other benefits derived from such status.

Termination of Agreement

The agreement will terminate at the end of the specified period, or either party may terminate this agreement after 15 days written notice to the other of intent to terminate. The parties shall deal with each other in good faith during the 15-day period after any notice of intent to terminate has been given.

Attachment A, “Drug Testing Procedure”, is integral to this Memorandum of Understanding.
