Staff Contributory Top-up Medical Insurance Scheme ("the Scheme")

The University has appointed Bupa as the underwriter of the Top-up Medical Insurance Scheme commencing from September 1, 2014. The scheme is set up to meet colleagues' need to obtain additional medical coverage for themselves and their dependants on top of the medical benefits provided by the University. Details of the Scheme can be obtained at the University Health Service (UHS) Website:

The Scheme

2. The Scheme aims at covering most of the shortfall between the benefits entitlement under the University's Staff Medical Benefits Scheme and the actual costs of hospitalisation. A summary of the major features of the Scheme is listed below:

? Annual premium: The premium rates are linked to age and benefits level. Details can be viewed at UHS website (). For the policy year from September 1, 2015 to August 31, 2016, Bupa offers a special 30% discount on the annual premium to staff and a 10% discount to dependants for their first year enrollment.

The premium is calculated from the date the member joins the Scheme on a pro-rata basis. There is no premium refund should the member leaves the Scheme before the expiry of the policy.

? Annual coverage: Up to a maximum of HK$750,000 per person for meeting eligible medical expenses in excess of the maximum reimbursement limits set out in the University's Staff Medical Benefits Scheme. The Schedule of Benefits can be viewed at UHS website: ()

? Annual excess: HK$7,500 per person (for appointees eligible for Plan 1) HK$3,750 per person (for appointees eligible for Plan 2)

? Eligibility:

Appointees who are eligible for either Plan 1 or Plan 2 hospitalization benefits under the University's Staff Medical Benefits Scheme.

? Dependants:

? Participation:

Spouse and dependent children, as defined by the University's Terms of Service, must enrol at the same time as the staff member unless they are residing or receiving education overseas or they are covered by another medical insurance scheme operated by the spouse's employers.

Participation is voluntary. New members of staff must enrol within 1 month of joining the University. Late participation will not be accepted.

Members aged 65 or above are required to complete a Health Declaration Form, obtainable from the UHS website:

? Pre-existing

Any pre-existing medical conditions will not be covered by the Scheme for

conditions:

the initial one-year period.

? No claims Bonus: A no-claim-bonus equivalent to 10% of the premium will be given to

members who submit no claim during the entire policy year.

3. This Scheme is tailor-made to meet the shortfall of the University's medical benefits scheme and the premium is set at a very competitive level. For a family of four, the annual premium is about

$6,500 (for 2 class private ward). The Plan will provide an additional annual coverage of up to $750,000 per person for meeting eligible medical expenses which exceed the University's reimbursable limit.

Additional Features

4. Bupa offers members the additional option of transferring to the Hospital and Surgical Benefit of Bupa CarePro Scheme at ward level without any underwriting approval or medical examination upon leaving the University or cessation of their hospitalization benefits at the University, provided members submit the application within one month before the cessation of their hospitalization benefits.

To enrol for the Scheme, please complete and return the Application Form (obtainable at the

following link: ), to the University Health

Service. For applicant aged 65 or above, please attach the Health Declaration Form to the application.

Please write "Top-Up Medical Insurance Scheme" on the envelope. New appointees must apply

within one month of joining the University. Late applications will not be accepted.

6. Colleagues can view their medical benefits entitlement by logging in "My Page" under HKU Portal. First, login to HKU Portal at using their HKU Portal UID and PIN. In "My Page" under HKU Portal, click "Self Service", select "Human Resources,", then click "Personnel File" In "Personnel File", select `Benefits", then click "Details" under Employment, the medical benefits entitlement is displayed in the Benefits Summary page.
