LeDeaux, MD who has been involved in a variety of transformative primary care delivery projects including: the Patient Centered Medical Home, Open Access Scheduling, Patient Activation/Self-Management in Chronic Illness, Informed/Shared Decision-Making, and Whole Health. Dr. LeDeux will be talking about the whole health model, which is transforming care as it has been known in the VA system both locally and nationally.Maureen Khung, Associate Director Field Implementation, VHA Office of Patient Centered Care & Cultural Information is responsible for developing and deploying strategies that support health care system transformation and field implementation of the Whole Health approach. Most recently, she designed and implemented national strategies for field implementation of personalized, proactive patient centered care and transformation to a Whole Health System. Ms. Khung will speak on key aspects of Whole Health and how this care approach engages veterans to take an active role in their health care. Two local Veterans will also be speaking about their journey with Whole Health.There will be afternoon break-out sessions for Monroe County, Seneca & Ontario Counties and Livingston, Wayne and Yates Counties. These sessions will be comprised of a panel of professionals from Veterans Service Agencies and other community service providers who will discuss the mission of their agencies, what services they provide for veteran’s as well as other community members, what successes their organization has with providing services and Whole Health to the community with a focus on veterans, what barriers have made it difficult to provide care and how these agencies can interface to break down barriers to provide true Whole Health Care to those they serve.
