If a formal solicitation is released, it will be posted via the Federal Business Opportunity ( OF THE REQUIREMENT:STATEMENT OF WORK:The Department of Veterans Affairs Birmingham VA Medical Center- Nuclear Medicine Service requires the commodity of Miscellaneous Radiopharmaceuticals necessary for the diagnosis and treatment of various diseases and disorders in Veterans.1. Contractor shall provide all labor, compounding, supervision and transportation necessary to provide daily deliveries to the Department of Veterans Affairs, Nuclear Medicine Services Birmingham VAMC. As requested. 2. Contractor shall ensure all shipping containers must be free of radioactive contamination both inside and outside, shall meet and be in compliance with all state and federal regulating agencies including but not limited to Department of Transportation (DOT), VA National Health Physics Program and Nuclear Regulatory Commission (NRC) as required by law.3. Contractor shall meet all Nuclear Regulatory Commission, National Health Physics Program (NHPP), Department of Transportation, FDA, OSHA, EPA, USP 797, USP 823, NRC and all other agency rules and regulations (Federal and State), as well as any applicable Joint Commission Requirements.4. Contractor shall be licensed by the Nuclear Regulatory Commission and be regularly established in the business of providing radiopharmaceuticals. Offeror shall provide copies of licenses and certifications and radioactive material permit along with proposal to the Contracting Officer.5. Radiopharmaceutical orders must be within +/- (10) ten percent (%) of the doctor’s order allowing only for correcting calibration date and time. All radiopharmaceuticals must be properly labeled on both the shipping container and the radiopharmaceutical container.6. No additional charges will be accepted and there is no limit on the number of deliveries made to the VA Medical Center7. If contractor cannot supply dose due to technical /mechanical problems failures, contractor is required to buy the requested doses from another source and transport to the Birmingham Veterans Affairs Medical Center, Nuclear Medicine Service.8. Contractor shall be able to provide service of all general & therapeutic Nuclear Medicine radiopharmaceuticals to Birmingham VA Medical Center.9. Contractor shall label all unit doses of delivered radiopharmaceutical with the amount, preparation time, expiration time, date etc.10. Contractor must supply with each shipment/order/delivery a system for “SPENT” (Used) syringes and needles that meets OSHA’s Biohazard Policy and regulations11. All packages containing radioactive material must be properly labeled on both the shipping container and the radiopharmaceutical container in accordance with title 49, Code of Federal Regulations Part 172 and 17312. Contractor shall perform all quality control procedures as required by Federal and State regulatory agencies. Quality Control results shall be furnished upon request. 13. Normal/Routine Ordering: Orders shall be placed by authorized VA personnel during the normal workday (7:30 am to 4:40 pm) and first delivery shall be received no earlier than 7:30 am the following workday to the Birmingham VAMC Nuclear Medicine Service. After award, the Contractor shall be furnished the name (s) of VA personnel that are authorized to place orders against the Contract. Orders shall only be place by authorized Birmingham VAMC personnel via telephone/fax or web ordering.Note: Doses pre-calibrated as 12;00 PM on the day of delivery or calibrated as requested and delivered during the hours of 7:30 am and 4:30 pm15. Normal/Routine Deliveries: The contractor shall provide routine delivery and pick-up, as required by Birmingham VAMC Nuclear Medicine Service.Delivery orders shall include the following information:Contract item (s) and quantity,Time (s) od delivery,Applicable VA purchase order16. As required by HIPPA, the Department of Health and Human Services (HHS) has promulgated rules governing the security and use and disclosure of protected heath information by covered entities, including the Department of Veterans Affairs (VA). In accordance with HIPPA, the Contractor shall be required to enter into a Business Associate Agreement (BAA) with the VA.Contractor shall be required to take annual HIPPA and Privacy and Security Training on TMS.Schedule of Supplies and ServicesThe Department of Veterans Affairs, Birmingham Veterans Medical Center Nuclear Medicine Service requires the contractor to provide the commodity of radiopharmaceuticals (misc).
