Documentation

? Are skin tests signed or initialed? ? Are PFT's signed or initialed? ? ROS and past family and social

history obtained by someone other than the provider? ? EHR's ? if scanned in - initialed first? ? Are there separate reports for challenges, nebulizer treatments, MDI instructions? ? How often are the allergy injection records reviewed and documented as reviewed?
