Connecting, Growing, Serving, and Going from HD to the Community!2257425204470 Job Description: Prevention Coordinator Agency: Higher Dimension ChurchDivision: Office of Community Empowerment (OCE)Department: Youth & Family ServicesClassification : Full-Time, Exempt – Project Employee* Job Location: Westwood Community/Alief ISD Southwest Harris County, Houston, Texas Summary of the Prevention Coordinator Responsibilities: Under the supervision of the OCE Overseer, the Prevention Coordinator will provide education and prevention services to families and youth. Services will target the prevention of youth problem behaviors including substance abuse, STD and HIV transmission, teen pregnancy, unsafe sex practices, gang involvement, violence and academic failure. The Prevention Coordinator shall operate in accordance with the policies set forth by Higher Dimension Church. The Prevention Coordinator will work with and serve the Westwood and Alief Independent School District communities.The Prevention Coordinator position is a full-time, exempt project employee and contingent upon availability of project and other project funding sources. Job performance will be reviewed quarterly by Higher Dimension Church’s Board of Directors. Qualifications: Minimum of 1 – 3 years of applicable experience.Masters degree in psychology or a closely related field preferred.Supervisory training or other leadership position with at least one year experience is a plus. Demonstrate effective verbal and written communication skills, independent working skills and the ability to organize and prioritize multiple tasks.Possess group facilitation skills. Including but not limited to facilitating afterschool, evening, and/or Saturday groups; teaching from an approved/adopted curriculum, and developing lesson plans.Ability to integrate and monitor prevention program trends and data.Demonstrated ability to take initiative in problem resolution in a professional and dependable manner.Exhibit and maintain professional work attire.Awareness of the disease of addiction and prior training in prevention preferred.Employee must be free of chemical dependency for at least 2 years prior to employment. Testing will be a portion of the hiring process completed by Higher Dimension Church designee.Physical Ability- able to lift/move equipment and supplies – up to 20-50 pounds – for presentations within the community. Valid Driver’s License.Reliable Transportation, for required travel throughout Westwood Community/Alief ISD, Southwest Harris County, Houston, Texas. Preferred Qualifications: Experience in classroom delivery and facilitation. Ability to work with diverse populations. Familiarity with local community; experience with prevention efforts in this community. Knowledge of Strategic Prevention Framework or other evidence-based practices.Duties and Responsibilities: Coordinate and facilitate the delivery of Substance Abuse and HIV Prevention education and activities. Assist in the coordination of activities and programs, delegation and follow up of training of community members/sector reps/community partners in fulfillment of education and prevention objectives/measures. Work with community partners and key stakeholders at the local and state level in implementing project objectives in accordance with the terms of funding agents.Work with all communities within Westwood Community/Alief ISD, which will require travel within the neighborhood boundaries to facilitate project goals. Responsible for timely completion, collection, storing, and compiling of project documentation and reporting back to Overseer, Case Manager and Sector Representative Group for final approval/review prior to submission. With the assistance and collaboration of Westwood Community/Alief ISD Board/Coalition, implement, facilitate and revise as needed the comprehensive community plan to address youth substance abuse in Westwood Community/Alief ISD. Coordinate efforts within Westwood Community/Alief ISD and with other communities as deemed appropriate to complete all milestones and produce key products as outlined in the SPF guide and as outlined in the Westwood Community/Alief ISD Action Plans. Engage, support and mobilize key stakeholders and sub-committees in face to face and virtual (webinar, tele-conference, etc.), communication/education efforts.Be aware of and involve ethnic, racial minority and underserved populations in the implementation of the project. Attend and participate in substance abuse and HIV prevention seminars and other project provided learning events with other Community Sector Representatives, as required by the project, ongoing online/virtual learning events. (Room and Board costs covered a per diem set by project guidelines/budget) Participate in city wide and/or community wide planning meetings as appropriate.Responsible for scheduling, organizing, developing agendas, leading, maintaining lesson plans and providing feedback to the Case Manager and at Sector Representative meetings. Ensure that strategies planned support sustainable evidence-based prevention strategies that positively impact community level policies, practices, planning, and programs. Ensure that adequate data collection for evaluation purposes, consistent with reporting requirements for the National Outcome Measures, community level evaluation and program evaluation. Projects may require the community to include pre, post, and six-month follow-up data collection for program level evaluation and other evaluation tools as identified through the project process.Work in partnership with The Council on Alcohol and Drugs Houston (The Council) and/or Texas Commission on Alcohol and Drug Abuse (TCADA) and agree to share new learning’s from this project experience with other communities across the state both in face-to-face and virtual gatherings. Bring information and direction back to Westwood Community/Alief ISD Coalition and Sector Representatives group. Contribute expertise and experiences in collaborative efforts with other Prevention Coordinators as appropriate.Other duties as directed by Overseer directly related to coordination of the Substance Abuse/HIV Prevention initiative.Maintain weekly office hours, contact information (address, e-mail, office phone number, cell phone number, etc.) to Westwood Community/Alief ISD Coalition Sector Representatives and other Westwood Community/Alief ISD initiatives. Provide completed bi-weekly timesheet to the Office of Community Empowerment accountant for signature prior to turning timesheet in to the Overseer for payroll purposes. All requisitions for fund distribution must be signed by the Overseer prior to presentation to the Higher Dimension Church Project Team accountant. A copy of all requisitions for budget verification purposes will be maintained by the Higher Dimension Church Project Team accountant. Hours:Full-Time – 40 hours/week – Office hours will be conducted from designated Higher Dimension Church Office of Community Empowerment facility.Some evenings and weekend hours are required to meet with local community coalitions/partners or to facilitate travel. ................
