Hello, my name is ____ and I am calling from _____.Hospice program. I just want to make sure you have everything you need for______ (this evening, this coming weekend, etc.)
1. Do you know if you have enough of your medications to last through the weekend?
2. Are you running low on any supplies?
3. Is the medical equipment you have at this time sufficient?
4. Sometimes we find that people feel anxious going into the weekend. Often we notice there are concerns or thoughts that become worrisome that develop along the way. Remember we have skilled team members available to assist with these feelings, either for you, your loved one or other family members (depending upon who you are speaking with.) Would it be helpful for you to have a visit from one of our staff?
5. We also have a team of highly trained and very supportive volunteers. Would it be helpful to have a volunteer come out so that you can feel comfortable getting out of the house, or to just have some company or someone to talk with (again, depending upon who you are speaking with)?
6. Can you think of anything we are missing at this point? Is there something we could be doing better or something you had hoped for but are not receiving?
