SUMMARY INFORMATION

Post: consultant cardiologist with sub-specialty interest in cardiac imaging

Base: glasgow royal infirmary

Applications are invited for the above post within NHS Greater Glasgow and Clyde and the West of Scotland Regional Heart and Lung Service. The base site for this post within NHS GG&C is Glasgow Royal Infirmary.

The successful candidate will be expected to provide a full range of consultant services as agreed with colleagues and management, including out-patient and in-patient work. Out-patient work will be undertaken at Glasgow Royal Infirmary and Stobhill Ambulatory Care Hospital, and will include combined new and return clinics in diabetes and endocrinology. The appointee will also contribute to acute cardiology receiving, the consultant on-call rota and to general cardiology cover at weekends.

Interventional cardiology services, including cardiac and thoracic services, are fully operational within the West of Scotland Regional Heart and Lung Service based at the Golden Jubilee National Hospital. Jointly, the well established medical cardiology services within NHS GG&C and the GJNH provide a high quality clinical service in all areas of cardiology including full range of cardiac catheterisation procedures, brady and complex pacing device implantation, electrophysiology, general cardiology, heart failure and adult congenital heart. Base sites for the successful appointee joining the active cardiology services, and the potential requirement to work at the GJNH on a sessional basis, will be determined by post and area of specialist interest. The successful appointee will play a role in expanding and developing services – our intention is to create a single system for patients and staff across NHSGG&C and GJNH. In addition to the identified specialist interests, the successful appointees will share the general cardiology and general medical workload with other colleagues across NHSGG&C and GJNH sites. There is a substantial academic presence and very good links with Glasgow University in all areas of the services provided. A number of consultants have national and international reputations.

Applicants must have full registration with the General Medical Council, a licence to practice and be eligible for inclusion in the GMC Specialist Register. Those trained in the UK should have evidence of higher Specialist Training leading to a CCT in Emergency Medicine or eligibility for specialist registration (CESR) or be within six months of confirmed entry at the date of interview. Non UK applicants must demonstrate equivalent training and have some experience of working in the NHS.

Person Specification

The successful candidate should have the MRCP and be within 6 months of obtaining a CCT in cardiology from the date of Interview. You should have extensive training in general cardiology and cardiac imaging, particularly echocardiography and cardiac CT. Training in cardiac MR would be desirable.

BACKGROUND INFORMATION

Living & Working in Glasgow

The night life and restaurants are renowned and its opera, theatres, art galleries and museums offer plenty of cultural stimulation. It’s shopping and retail sector is regarded as one of the best in the UK and arguably second only to London.

Health Board

NHS Greater Glasgow and Clyde is the largest health board in Scotland and serves a population of 1.1 million people. There are 8 hospitals north of the River Clyde, all with teaching responsibilities: Glasgow Royal Infirmary (including Princess Royal Maternity Hospital), New Stobhill Hospital, Lightburn Hospital, Glasgow Dental Hospital and School, Western Infirmary, Gartnavel General Hospital, Drumchapel Hospital, and Blawarthill Hospital. Three hospitals are situated in the south side of the city: Southern General Hospital, Victoria Infirmary and Dykebar Hospital. The acute Hospitals sites in Clyde are the Royal Alexandra Hospital, Inverclyde Hospital and the Vale of Leven Hospital. The Golden Jubilee National Hospital (GJNH) in Clydebank is the setting for the new West of Scotland Heart and Lung Centre. The centre brings together, on a single site, cardiothoracic surgical services for the West of Scotland and is the dedicated unit for all interventional cardiology including primary PCI for NHS Greater Glasgow and Clyde and most surrounding Health Boards.

Acute Services Division

The Acute Services Division is the largest group of adult acute hospitals in Scotland. It enjoys close links with Glasgow’s three universities and makes a significant contribution to teaching at both undergraduate and postgraduate level. Research also has a high profile within the organisation. Excellent education facilities are also provided at Glasgow Royal Infirmary and the new ambulatory care hospital at Stobhill Hospital. The Service in North Glasgow presently employs more than 14,300 staff serving a core catchment population of 560,000.

Glasgow Royal Infirmary

Glasgow Royal Infirmary (GRI) is one of the major teaching complexes of the University of Glasgow. It provides the Accident and Emergency service for the North and Eastern districts of Glasgow and has inpatients beds in general medicine, medicine for the elderly, general surgery, orthopaedics, plastic surgery and obstetrics and gynaecology. There are also beds in intensive care, high dependency, and coronary care. Following the closure of Stobhill hospital at the end of March 2011 all medical beds, with the exception of some long-stay care of the elderly beds, are on the GRI site.

GRI is well equipped for undergraduate and postgraduate teaching. There are academic units in Anaesthetics, Cardiology, Human Nutrition, Medicine, Surgery, and Obstetrics and Gynaecology.

There is a 40 bedded cardiology ward (ward 43) with 20 acute cardiology beds and a coronary care unit (CCU). Those needing admission for a primary cardiological problem go from an Acute Assessment Unit (AAU) or A&E to the acute cardiology beds in ward 43 or CCU. The cardiologists also provide a daily consult service to patients needing review in the Acute Medicine Unit (AMU). At GRI medical emergencies are admitted through AAU or through Accident & Emergency (A&E) to an AMU.

Acute admissions to medicine range from 45 to 70 per day. Patients requiring a short stay will be discharged from AMU/acute cardiology beds. Those requiring longer stays are transferred ‘downstream’ to medical wards (non acute cardiology bed in ward 43 for cardiological problems).

Cardiology.

The Cardiology unit is accommodated in CCU and Ward 43 and has capacity for 54 inpatients. Consultant cover is provided by Drs Hogg, Goodfield and the newly appointed EP consultant. It is envisaged that the successful candidate will contribute two ward rounds per week.

Cardiology out-patient care is delivered in the out patient department at Glasgow Royal Infirmary and in the New Stobhill Ambulatory Care Hospital (ACH).

Echocardiography. The Kerry-Jane Hogg Heart Unit at Stobhill ACH has 2 large dedicated echocardiography rooms each with a Philips iE33. There are 1.5 dedicated echo technician WTE’s performing 13 sessions of transthoracic echocardiography per week, amounting to 2775 echocardiograms / year. There are also 2 dedicated consultant sessions per week in which OP TOE, DSE and advanced echo are performed. The GRI Cardiology Department has 2 dedicated echo rooms each with a GE Vivid7. There are 2.7 dedicated echo technicians performing transthoracic echocardiography per week, amounting to 7112 echocardiogram / year.

Cardiac CT. Cardiac CT in GG&C is performed either at the at GRI - 1 dedicated session of 7 OP CT's and further ad hoc IP CT's per week. These are performed on a 64 slice Toshiba machine and workstation.

Cardiac MR. Current adult CMR in GG&C is performed at Stobhill (20 studies per month) where there is a 1.5T Philips Achieva. At the end of March this year a new 1.5T Philips Ingenia will be installed at GRI with specialist cardiac protocols installed providing potential for performing stress perfusion studies.

All cardiothoracic surgery and interventional cardiology is carried out at the West of Scotland Heart and Lung Centre in the Golden Jubilee National Hospital in Clydebank on the outskirts of Glasgow. This is a dedicated regional unit for interventional cardiology including primary percutaneous coronary intervention (PCI). In 2009 the hospital carried out approximately 2600 PCI procedures (including 700 PPCI’s), 400 device implants (CRT and ICD) and 350 electrophysiology interventions. With this array of specialist cardiopulmonary services the GJNH is one of the largest heart and lung centres in Europe.

Duties of the Post

This is a new consultant post and the successful candidate will be expected to provide a full range of consultant services as agreed with colleagues including in-patient management of patients admitted to the cardiology wards. The pattern and frequency of these duties will be finalised following appointment to the post.

It is envisaged that outpatient work will be undertaken both at GRI and at Stobhill ACH. It is expected that the successful candidate will provide three clinics per week to support the unit in striving to meet the 12 week waiting list target. The development of a subspecialty interest will be encouraged and supported. In particular, the department has a need to expand its imaging capacity in providing echocardiography and CT coronary angiography, however, other interests could be accommodated.

Consultants are expected to participate actively in audit, and initiate and supervise audit projects related to specialty interest. Participation in national audit is encouraged. One SPA session is allocated for audit, appraisal, job planning and professional development. A split of 9:1 between direct clinical care PAs and supporting professional activities is now the advertised standard for all new consultant job plans in Scotland.   The one SPA minimum will reflect activity such as appraisal, personal audit and professional development occurring outside study leave time.   Once the candidate has been appointed more SPA time may be agreed for activities such as undergraduate and postgraduate medical training which takes place outside direct clinical care, as well as research and/or management.  These activities must be specifically and clearly identified and be agreed with the candidate and desired by the directorate.

Cardiology is part of the Emergency Care and Medical Specialties Directorate, NHS Greater Glasgow and Clyde. Mr Grant Archibald is the Director of Emergency Care and Medical Specialties across NHS Greater Glasgow and Clyde, and is supported by Dr David Raeside, Associate Medical Director. The Clinical Director for Cardiology is Dr David Murdoch, specific management responsibility for Cardiology is by Mr Alan Hunter, General Manager Cardiology, NHS Greater Glasgow and Clyde.
