Procedure for Exiting WIA Clients

Soft Exit (Client is employed and the exit code is UE)

1. Verify employment

Order of preference

a. Pay Stub

b. Employer Verification Form

c. Attachment B (speak with employer or customer)

d. Q01 Screen or TAXIS

2. Complete MIS forms to leave all activities

a. No activities can be open

b. EMJ needs to end on the exit date

c. No activities can end after the exit date

3. Complete Exit form with UE code and fill-out completely

4. Turn in all of the leave MIS forms and Exit form to Seth (staple together by client) and keep copies in customer’s file

Negative or Neutral Exit (Client is unemployed and the exit code is NOT UE)

1. Complete MIS forms to leave all activities

a. No activities can be open

b. HPO or other code needs to end on the exit date

c. No activities can end after the exit date

2. Complete Exit form with OL, OM, OH, or OS code and fill-out top part only

3. Turn in your file (with all of the leave MIS forms and Exit form stapled together on top) to Seth (keep copies in customer’s file)

Hard Exit (Client was exited as a UE and one-year retention is over)

1. Complete Hard Exit Form

a. Check the box for employed 1st Quarter after Exit

b. Check the box for employed 2nd and 3rd Quarter after Exit if you verified that this is true from client, UI Q01, or SKIES

c. Fill-in the amount for 2nd and 3rd Quarter after exit earnings

2. Turn in Hard Exit Form to Vonita

3. Turn in your file to Seth (place copy of Hard Exit Form and copy of UI Q01 or TAXIS Screen in File)
