11239507620000Healthcare for Homeless Veterans Community Resource GuideRichland, Lexington and Greenwood CountiesHealth Care for Homeless Veterans Program (HCHV)The Healthcare for Homeless Veterans (HCHV) program seeks to establish contact with homeless/at-risk Veterans to facilitate access to treatment services and available resources in an effort to improve not only housing concerns, but the issues (e.g. medical, psychiatric and/or substance abuse problems) that so often underlie a Veteran’s homelessness. The central goal of the HCHV Program is to end homelessness among Veterans. The HCHV Clinic provides assessments and referrals to community based VA services. These include: housing, homeless prevention, employment and training services, health care, benefits, substance abuse treatment and mental health services. The HCHV Clinic is staffed Monday through Friday from 8:00 a.m. – 4:30 p.m. and is located inside of Building 106 on the 2nd floor.The HCHV Program provides access to several transitional and permanent housing programs, each with specific eligibility criteria. The transitional housing programs are located in Richland, Lexington and Greenwood counties. Program Entry Beds- Program provides case management services and short term housing (45-60 days) to individuals homeless men and women who are motivated and ready to participate in a structured program of recovery, including increasing their income and securing permanent housing. Transitional Housing units- Provides a three month to two year opportunity to fully address individual obstacles in order to achieve self-sufficiency. Convalescent Bed- Provide shelter to currently homeless men while they recuperate from physical illness or injury rather than discharge to traditional emergency housing facilities or the street.
