2019-2020 Financial Aid Award Declination/Revision Form

This form must be completed and submitted to the Financial Aid Office when reporting and/or requesting a change(s) that will affect a student's financial aid. Also complete this form if you wish to decline or revise all or a portion of the financial aid offered to you. If you do not return this form, we will assume that you accept your financial aid offer. Please refer to the Financial Aid Terms and Conditions for information about the types of aid that you have been awarded.
