There are two steps to complete your voluntary wellness incentive and earn a $300 reduction in

medical premiums for 2025.

Step 1: Take the health check survey

Step 2: Participate in a biometric screening

Important dates to know

? Complete both steps by September 30, 2024.

? The individual program screening option can only

be selected until September 25, 2024.

Additional information

? Free and available to full-time U.S. NXP employees.

? New hires with a start date in 2024 will automatically

receive the 2025 wellness incentive credit.

? Execs (G8 and above) can use their executive

physical to earn the wellness incentive. The physical

must take place between 10/1/23 - 9/30/24.

To access the survey:

Create or log in to your account at .

Navigate to the Health > Survey page.

You'll be redirected to where you can start your Health Assessment. Click Start to begin.

If you have a middle initial on your medical ID

card, you must include a space and your middle

initial along with your first name.

If you have a hyphenated last name, you must

remove the hyphen when getting registered for

the first time on Virgin Pulse.
