GENERAL INSTRUCTIONS

Responsibility

The Dirouhie, Pokh Dirouhie, Tbrouhie and Kantzabedouhie, who served during the fiscal year, should prepare the Report.

Fiscal Year

All information provided must be for the fiscal year covering April 1, 2019, through March 31, 2020.

Forms – General Information

You may want to make additional copies of the forms to use as working copies. If you received the forms via email, you will need to save a copy to your computer before entering and saving the information.

For those requiring the reports via US mail, you may use a paper copy.

Dues Assessment, Initiation Fees and Charitable Donations

Please remit with your Annual Report the:

▪ Dues assessment of $30 per member based on your membership count as of December 31, 2019 (Note: There is an exemption for dues assessments for new members who joined between January 1, 2020 and March 31, 2020)

▪ Initiation fees of $8 per new member who joined any time between April 1, 2019, through March 31, 2020

▪ Charitable donations total from all Charitable Donation Forms submitted.

Only one check is necessary. Checks should be made payable to Daughters of Vartan Grand Council and dated on or before March 31, 2020.

Submission of Completed Reports

You may submit the reports via US mail or email. Email is preferred. (On Original form-Printed)

Signatures are not required for completed reports sent via email. Simply type in the name of the preparer.

One copy of the “Annual Report” and the “Delegates Form” must be sent to each of the following:

Due Date

Annual Reports sent via US mail must be postmarked no later than April 24, 2020. Annual Reports sent via email must be sent by April 29, 2020. It is imperative that all reports reach Avak Khorhourt by the due date. The convocation reports and tax returns cannot be completed on time if the reports are late. Any report not received by April 29, 2020 will not be included in the convocation booklet. It must be on the original form, printed).

The number of Delegates from an Otyag is determined by the number of members as follows:

Form 990-N (e-Postcard)

Annual Electronic Filing Requirement for Small Exempt Organizations

IRS Form 990-N (also called “e-Postcard) must be filed by each Otyag for the prior fiscal year-ending March 31.

It can only be filed electronically (on computer), and is due by August 15th. There is no paper form.

You will need the following information to file:

▪ Tax Exempt Number

▪ Tax Year (e.g., April 1, 2019 – March 31, 2020

▪ Legal Name and Mailing Address

▪ Name and Address of a Principal Officer

▪ Confirmation that gross receipts are under $25,000

It is very important that you notify Avak Kantzabedouhie when you have completed filing of the e-postcard.
