You are to create a form using one of the following content areas:

← 2nd grade Science on Cells and Plants

← 3rd grade Math on Place Value and Decimals

← Pre-Kindergarten Shapes, Colors, Days of the Week, Numbers and Alphabet

← 1st grade English and Antonyms

← 5th grade Spelling and Homonyms

You need to first create a new folder on your flash drive saved as Elementary School. Your site will have 2 pages (homepage.html and practicetest.html).

Homepage Info

• You should create a very simple homepage for your class with

• Your name (as the teacher) and a picture

• The grade level you teach,

• Your daily schedule of classes lunch and recess (in 30 minute increments),

• The principal’s name and a picture

• Your classroom motto (example: Every Student. Every Day. College Bound. Go Vikings!)

• Include a link to the practice test from your homepage

• Include a link to any elementary school in the Lakeside feeder system

The form needs to be a test for your students. The test must include text boxes, radio buttons, checkboxes, drop down menus, a scrollable menu, and reset and submit buttons. You must have at least one of each type of question. In addition, the test must have at least 25 questions. Be sure to proofread so that there are NO typos. Search the internet to find specifics on what is taught in your content on that grade level.

Website Requirements:

In this site you must use all of the following that you have learned so far:

• Horizontal Rule with 1 attribute (size or color)

• Font Face and Font Color Changes

• Heading 1 and Heading 2

• Change Background color

• Bold, Underline, Italics

• Ordered List and Unordered List

• No more than 3 images

Be prepare to present your website. Due on October 3, 2016
