What is this test designed for? A nuclear stress test is a diagnostic test using a radioactive tracer to determine if the heart’s coronary arteries are providing adequate blood flow to the heart muscle. The test consists of two sets of pictures and a stress test, totaling 1 hour but you will be here approximately 3 to 4 hours. Most of the time will be spent in the waiting room, so bringing reading material is recommended.

A late arrival may result in the rescheduling of the test.

WHAT NOT TO DO:

• No caffeine containing products for at least 24 hours prior to the test. This includes all forms of coffee, tea, soft drinks, chocolate, and beverages labeled decaffeinated and caffeine free, herbals, Excedrin and/or Anacin.

• No eating or drinking 4 hours prior to test. A small amount of water is ok.

• No Persantine, Dipyridamole, Aggrenox, Aminophylline, or Theophylline 48 hours prior to test. Please continue all other medications unless otherwise directed.

• Additional Medications to be held/Other Instructions:

• No Beta Blockers 24 hours prior to Stress test.

Such as: Betapace, Sotalol, Lopressor, Metoprolol, Atenolol,

Zebeta, Bisoprolol, Timolol, Coreg, Carvedilol, Inderal,

Propanelol, Labetalol, Bystolic

• No Calcium Channel blockers 48 hours prior to Stress test.

Such as: Cardizem, Diltiazem, Verapamil, Carlan SR, Covera, Verelen, Cartia, Tarka

➢ Hold Nitrates also for 24hrs prior: Isosorbide (imdur) or (Isordil) Nitro patches, Ranexa

WHAT TO DO:

• Drink plenty of water the day prior to your test.

• Wear comfortable loose clothing and appropriate shoes for exercise. Please no metal buttons or zippers on shirts. For your safety, bare feet and sandals are not permitted.

• The department is kept very cool, please bring a sweater or blanket with you.

• Bring a snack and drink with you. You will be advised to eat after the test begins.

• Do not use body lotions or oils the day of your test.

• Women, please wear two-piece outfits. Please do not wear underwire bras.

• Patients taking insulin; take half of your morning dose.

Patients who weigh more than 250lbs. may require a 2 day protocol.

Females will need to sign a consent form stating they are not pregnant. If there is a possibility of pregnancy, we will request a pregnancy test be performed and will reschedule your appointment.

To provide good care and protect your privacy, family members will be asked to remain in the waiting room.

Procedure

Part 1: REST IMAGES

An IV will be placed in your arm. A small amount of a radioactive tracer will be injected through this IV. The radioactive tracer will not have any side affects. The technologist will ask you to wait 30 minutes before acquiring images, but during this waiting period you are encouraged to drink water. For the pictures, you will be asked to lie on your back with both arms above your head. A camera will rotate slowly across your chest taking approximately 20 minutes. You may breathe normally while the pictures are taken, but it is important to lie still.

Part 2: STRESS TEST You will do ONE of the following Two types of stress tests:

Option 1: ‭  EXERCISE

This stress test consists of walking on a treadmill. EKG electrodes will be placed on your chest and you will be connected to an EKG machine. The cardiovascular technologist will monitor you throughout the walking portion of the test. You will be asked to exercise until you reach your target heart rate or until you are unable to exercise further. The target heart rate is determined by your age. At this point, the nuclear technologist will inject a second dose of the radioactive tracer through the IV. You will be asked to walk for two more minutes after the injection.

Option 2:  LEXISCAN

This is a pharmacological stress test using the chemical lexiscan. EKG electrodes will be placed on your chest and you will be connected to an EKG machine. During the stress test, you will be given a 10-second infusion of lexiscan. Lexiscan dilates the blood vessels and you may experience chest pressure during the infusion, headache, or nausea. You will be monitored throughout this portion of the test and explained any side effects you may experience. After the lexiscan infusion, the technologist will give you a second dose of the radioactive tracer. Once the stress test is completed and symptoms dissipate, you will wait about 45 minutes before taking the final pictures (stress images).

Part 3: STRESS IMAGES

This portion of the test consists of a second set of pictures, stress images. You will be asked to lie on your back with both arms above your head. The camera will slowly rotate across your chest for approximately 20 minutes. You may breathe normally but must lie still.

Nuclear doses are ordered and timed for your appointment. Please give our office a 24-hour cancellation notice. A fee may be charged for the cost of the isotope if we do not receive notification of cancellation 24-hours prior to scheduled testing.
