INSURANCE REQUIREMENTS

The following is required by City of Miami Beach Risk Management Division:

Commercial General Liability on an occurrence basis, including products and completed operations, property damage, bodily injury and personal & advertising injury with limits no less than $1,000,000 per occurrence.

The City of Miami Beach must be listed as a CERTIFICATE HOLDER.

Certificate must show the City of Miami Beach’s address:

Miami Beach, FL 33139

The City of Miami Beach must be named as an ADDITIONAL INSURED.

This exact verbiage must be included under the section of Description of Operations/Locations/Vehicles/Special Items: The City of Miami Beach is included as an additional insured with respects to liability arising out of event name, dates including event load-in and load-out and event location.

Worker's Compensation as required by the State of Florida, with statutory limits, and Employer’s Liability with limit of no less than $1,000,000 per accident for bodily injury or disease.

(This is for companies that employ more than 3 employees).

If you do not carry workers compensation because you do not employ more then three employees, please check off the Workers Compensation Section when filling out the Indemnity Agreement.

Liquor Liability: limits $1,000,000 (if alcohol is served). If liquor liability as being provided by someone other then the applicant or on a separate certificate. The certificate must name the City of Miami Beach as an additional insured as well as a certificate holder.

Certificates must be dated within 90 days of the current date.

All policies must be issued by companies with A.M. Best’ rating of no less than B+VI, or authorized to do business in the State of Florida.

Contract grants a waiver of any right to subrogation which any insurer may acquire against the City of Miami Beach by virtue of the payment of any loss under such insurance. This provision applies regardless of whether the Entity has received a waiver of subrogation endorsement from the insurer.
