Program Description

This course is to prepare the adult cardiac medical and cardiac surgical nurse to take the AACN certification exam for Cardiac Medicine and/or Surgery Exam. The course focuses on adult cardiac abnormalities with a concentration on nursing assessment and medical/surgical interventions. Coronary artery disease, acute coronary syndrome, heart failure, endocarditis, valvular disease and aortic disease will be the major topics of discussion with related medical and surgical interventions. The course will also include a brief review of pulmonary, hematology, neurology, gastrointestinal and renal systems as they pertain to the cardiac patient.

Program Learning Outcomes

This program prepares the learner to: 1. Discuss and practice test-taking skills. 2. Review critical anatomy, physiology and pathophysiology of the cardiac system. 3. Define in detail: preload, afterload, contractility and cardiac output/index. 4. Relate pharmacological interventions for the medical and surgical cardiac patient. 5. Discuss the role of hemodynamics as a diagnostic tool for the development of

interventions for the cardiac patient. 6. Discuss significant assessment and diagnostic findings relevant to the cardiac adult

patient: specifically coronary artery disease and heart failure. 7. Discuss the clinical presentation as well as specific patient management modalities of

the cardiac medical problems. 8. Discuss the clinical presentation as well as specific patient management modalities of

the patient undergoing cardiac surgery.

Introduction

Module Description

This module will introduce the test format for the Cardiac Medicine Certification (CMC) Exam and Cardiac Surgery Certification (CSC) Exam. The American Association of Critical-Care Nurses (AACN) blueprints for both exams are presented in detail, and the application and renewal process are discussed.

Module Learning Objectives

This module prepares the learner to: 1. List the blueprint components for the separate examinations. 2. Describe how to apply for and schedule the examinations. 3. List how to recertify by examination and/or CEU requirements.

I. Introduction

A. Obtaining Application:

1. AACN 800-899-2226 or (go to "Certification" then "CMC/CSC") 2. Apply: Receive authorization to test (takes between 2-4 days to 2-3 weeks). 3. Schedule: Make appointment at nearest testing site: H&R Block, PSI testing center 4. You have 90 days to take the exam.

C. Requirements for the CMC Exam or the CSC Exam:

1. Current unrestricted RN license in U.S. 2. Current nationally accredited nursing specialty certification: CCRN, PCCN, CCRN-E,

CCNS, ACNPC, FNP, CEN, CRNFA or APRN a. CMC: 1,750 hours (875 hours in last year) in care of the cardiac medical patient-- NSTEMI/STEMI pre- and postcatheterization lab--acutely or critically ill cardiac patient b. CSC: 1,750 hours (875 hours within the first 48 hours postoperatively) direct care of acutely/critically ill adult patients during the previous two years c. Practice as an RN or APRN for at least 5 years with a minimum of 2,000 hours in direct care of acutely/critically ill adult patients, with 144 of those hours accrued in the most recent year preceding application. Of those 2,000 hours, 1,000 need to be in the care of acutely/critically ill adult cardiac surgery patients/or cardiac medical patients pre- and postcath lab--acutely or critically ill cardiac patient. d. Nurses serving as manager, educator (in-service or academic), APRN or preceptor may apply their hours spent supervising nursing students or nurses at the bedside. e. Nurses in these roles must be actively involved in caring for cardiac and/or cardiac surgery patients; for example, demonstrating how to measure pulmonary artery pressures or supervising a new employee or student nurse performing a procedure.

CSC Blueprint--90 Questions, 2 Hours a. 11% Cardiothoracic Surgery (procedures, coronary artery bypass grafting (CABG), thoracic b. 33% Complications c. 43% Therapeutic Interventions (antidysrhythmics, chest tubes, pulmonary vasodilators, blood products) d. 13% Monitoring and Diagnostics (arterial blood gases [ABGs], coagulation studies, capnography)

2. CMC Blueprint--90 Questions, 2 Hours a. 23% Cardiovascular Conditions (acute coronary syndrome [ACS], ST-elevation myocardial infarction [STEMI], non-ST-elevation myocardial infarction [NSTEMI], dysrhythmias, ventricular septal defect [VSD], heart failure [HF], valvular disease, tamponade) b. 23% Other Non-Cardiovascular Conditions (pulmonary, endocrine, hematology, neurology, renal, multisystem, behavior) c. 39% Therapeutic Interventions d. 16% Monitoring and Diagnostics

E. Recertification for the CMC/CSC

1. Candidates for Renewal: Same Guidelines a. Retake the examination b. Candidates must have 432 hours of clinical practice (with 144 accrued in past year). c. 25 CEUs Category A Clinical Judgement CERPs pertaining to cardiology/cardiac surgery
