Billing Procedure for CPT and CPR
CPT
Government will purchase kit/cube at the calculated price and be invoiced at time of shipment. Payment expected net 30 days, FOB destination. Efficiency of usage is the government’s responsibility. Calibrator/Control material and disposables as outlined in the no charge accessories lists provided will be shipped in on a monthly, bimonthly, quarterly or yearly basis. Quantities exceeding those listed on the attached pages will be the customer’s responsibility. Printer paper and cartridges are not included.

Basic CPT includes Reagents for Patient Testing, Calibrator Material, Control Material, Disposables and Reagents to perform Calibration and Control runs as outlined below. Assumptions:
This is a Cost per Test. The Cost per Test Result is based on allowing a maximum of 5 controls/shift/analyzer, calibration as
per manufacturers specifications/analyzer and 10% maximum repeats.
All disposable and buffer solutions included at no charge as per the quantity listed in catalog. FOB Destination for equipment, reagents and disposables. Additional charges will occur if the customer exceeds the maximum
allowed controls, calibrations and repeats.
Volume fluctuations will be assessed quarterly with changes in volume tiers occurring every 6 months. Minimum quantities per month apply.
Instrument provided may be new or refurbished.

This is a Cost per Reportable.
The Cost per Patient Result is based on allowing a maximum of 5 controls/shift/analyzer,1 calibration/6
months/analyzer and 10% maximum repeats.
All disposable and buffer solutions included at no charge as per the quantity listed in catalog. Reagents are shipped out at no charge and the customer is billed monthly based on the number of patients reported.
FOB Destination for equipment, reagents and disposables. Additional charges will occur if the customer exceeds
the maximum allowed controls, calibrations and repeats.

Volume fluctuations will be assessed quarterly with changes in volume tiers occurring every 6 months.
