|This AGREEMENT was made on___________________ , 2009 between ________________________ of_____PHC Reclamation____ and __Krista Collins |
|of___Kreative-Designs___. |
|Work desired will be referred to as “The Site” and will be acknowledged to be a contract between “KREATIVE-DESIGNS” (contractor) and (_____PHC |
|Reclamation_________ )( client) within this document. Please note that the “World Wide Web” is referred to as WWW. |
|Services To Be Performed. Contractor agrees to perform the following services for Client: Web Site containing ___twenty__ ( 20 ) pages. Contractor will |
|provide all aspects of the Site including Site design, registering the domain name for the client (if necessary), provide company for hosting (if necessary), |
|maintenance of The Site if needed, and any other designs the client may desire for The Site. |
|Time For Performance. Contractor agrees to complete the performance of these services on or before August 31 , 2009. If for any reason this time frame |
|becomes altered due to complications beyond the contractors abilities, contractor will notify client immediately. |
|3. Payment. In consideration of Contractor's performance of these services, Client agrees to pay Contractor as follows: |
|Domain Name registration: N/A |
|Hosting cost: N/A First year is free. Every year thereafter cost will be determined by the hosting company. 2009 costs are $9.95 a month. Th is fee may be |
|invoiced monthly or yearly. Clients choice |
|( Be aware that the cost may be higher at a future date due to increase in company prices. Client will always be notified of any changes.) |

|Web Site Design and Implementation of such design: ___twenty____ ( 20 ) page/s for $__2000.00__ . Price includes the design of The Site. A main page and |
|additional page/s consisting of: |

|Any additional page/s, if desired in the future will be an additional $100.00 per page. |

|Minor verbiage changes (a few words here and there) will not be charged. |
| |
|Other changes or additions to the initial 20 pages: A minimum of $20.00 an hour will be charged. |
| |
|Any special graphics, logos or designs, if desired, created by contractor will be an additional cost which will be determined and agreed upon by both parties in|
|writing. |
| |
|A preliminary design of The Site will be done prior to submission to the WWW and approval will be agreed upon by both parties. |
| |
|$200.00 of the cost of The Site is due upon signing of the contract as a good faith estimate. |
| |
| |
|The remaining costs will be due upon completion of The Site, in which The Site has been approved and agreed upon by both parties. The remainder will be invoiced|
|and paid according to both parties agreement. |
|Please note that in the event The Site cannot be agreed upon by both parties, and all changes requested by the client have been made by the contractor, domain|
|name costs and hosting fees may not be refunded. These costs are paid to outside companies and cannot always be returned. All efforts by contractor will be |
|made to refund this amount but are not guaranteed. |
| |
|Design fees are non-refundable and will be kept by the contractor. |
| |
|If both parties cannot agree on The Site created, The Site will be removed immediately from the WWW. |
|There may be a slight delay due to the clearing of a check by the financial institution of the contractor. |
|Upon which the check clears, services will resume as normal. |
|Invoices. Contractor will submit invoices via E-mail and/or regular mail for all services performed. |
|Independent Contractor. The parties intend Contractor to be an independent contractor in the performance of these services. Contractor shall have the right to |
|control and determine the method and means of performing the above services; Client shall not have the right to control or determine such method or means. The |
|Site, as far as the design is considered, remains the property of Kreative-Designs. |
|Any duplication, copy or reproduction under another name, through a different service representative or by any other means will result in lawful action. The |
|design is considered a piece of art and therefore remains property of kreative-designs. |
| |
|Other Clients. Contractor retains the right to perform services for other clients under Kreative-Designs. |
|Assistants. Contractor, at Contractor's expense, may employ such assistants as Contractor deems appropriate to carry out this agreement. Contractor will be |
|responsible for paying such assistants, as well as any expense attributable to such assistants, including income taxes, unemployment insurance, and social |
|security taxes. |
|Equipment and Supplies. . Contractor will provide all equipment, tools, and supplies necessary to perform the above services for creation and design of The Site|
|with the exception of any pictures, business logos or personal graphics the client desires added to The Site. The contractor understands that any logos remain |
|the property of the client and any duplication for reasons other than the design of The Site or for portfolio reasons are prohibited. |

|10. Request of Contactor. Contractor requests the permission to display a logo or link (clients choice) in a discreet area on The Site as a display of the |
|creator of The Site. The client may choose how this can be displayed and it is greatly appreciated by the contractor. |
| |
| |
|In Conclusion. This contract contains the entire agreement between the parties and may be amended only by written instrument signed by both parties. By signing |
|this agreement you are signing a legal and binding contract and any changes can only be made between the parties herin. |
