The enclosed form serves as the basis of that evaluation. Because only you know all that you have done, we ask that you complete the form (save as filename “Last name, First name IO Student Evaluation 2020-21”), and email it along with a current copy of your vita to your Major Professor and to Laura Pierce lpierce@usf.edu no later than Wednesday, April 21, 2021. The I-O faculty will meet to review and evaluate all students. Keep in mind that this is a requirement and is not optional.Please fill out all the items that apply by using the electronic version of this form. You will receive a copy of this via e-mail. There is also a downloadable copy on the department website Resources>Policies and Procedures>Students, in the Evaluation and Progress section at this link .
