Scope and Administration

Scope of Accreditation

(Insert name of Corporation) herein referred to as “The Corporation” will administer the Safety Codes Act (SCA) including all pursuant regulations applicable to the following indicated discipline(s), on properties owned by or under the care and control of the Corporation, within the Corporation’s industrial facilities (insert name of Municipality or as identified in Schedule A attached).

The Corporation will ensure any accreditation overlap issues, as outlined in the Safety Codes Act are addressed.

Building

All parts of the Alberta Building Code.

Electrical

All parts of the Canadian Electrical Code and all parts of the Code for Electrical Installations at Oil and Gas Facilities, and/or

All parts of the Alberta Electrical Utility Code.

Plumbing

All parts of the National Plumbing Code of Canada, applicable Alberta amendments and regulations, and Private Sewage Disposal System Regulation.

Gas

All parts of the Natural Gas and Propane Installations Code and Propane Storage and Handling Code, Code for Field-related Components on Appliance and Equipment, and Compressed Natural Gas Fuelling Stations Installation Code, excluding the Installation Code for Propane Fuel Systems and Tanks on Highway Vehicles and the Natural Gas for Vehicles Installation Code Part 1, Compressed Natural Gas.

Fire

Alberta Fire Code Administration: (one is mandatory)

All parts of the Alberta Fire Code, or

All parts of the Alberta Fire Code except for those requirements pertaining to the installation, alteration, and removal of storage tank systems for flammable liquids and combustible liquids regulated under the Alberta Fire Code (see details in Appendix E).

Fire Investigations: (mandatory)

Cause, origin, and circumstance determination.

Fire Prevention Programs: (mandatory)

Pre-planning fire prevention (see details in Appendix E).

Management Policy on Adherence to the Uniform Quality Management Plan

The Corporation is responsible for the administration, effectiveness, and compliance with this Uniform Quality Management Plan (UQMP) that includes, Schedule A – Scope and Administration, and Schedule B - Uniform Service Delivery Standards.

The Corporation will provide services under Schedule B – Uniform Service Delivery Standards through their own staff or one or more accredited agencies. When providing services through an agency(s), the Corporation will contract with the agency(s) to provide services in accordance with Schedule B – Uniform Service Delivery Standards. The Corporation is responsible for monitoring the contracted agency’s compliance with Schedule B – Uniform Service Delivery Standards.

The Corporation recognizes that the Safety Codes Council (SCC) or its representative may review/audit for compliance to this UQMP and will give full cooperation to the SCC or its representative in business related to the administration of the Safety Codes Act (SCA) including the conduct of reviews/audits. The Corporation will implement the recommendations of the reviewer/auditor.

The Corporation will encourage and maintain an atmosphere that supports objective and unbiased decisions. All Safety Codes Officers (SCO’s) working under this UQMP have the ability and opportunity to make decisions relative to compliance monitoring independently, without undue influence of management.

The Corporation is responsible for the effectiveness and conformance with this UQMP and will ensure that staff are adequately trained and prepared for its implementation. The Corporation has identified a QMP Manager who is responsible for the administration of the UQMP.

The Corporation, in the event that it ceases to administer the SCA for any new thing, process, or activity under the SCA, retains responsibility for completion of the services initiated under the SCA while accredited.

The Corporation recognizes that failure to follow this UQMP or the recommendations of the auditor may result in suspension or cancellation of the Corporation’s accreditation.

Management and all officers, staff and SCO’s of the Corporation, including engaged accredited agency(s) shall respect and comply with the policies and procedures covered in this UQMP; and will adhere to SCC policy.

Declaration of Status

The Corporation, officers, staff, and its SCO’s, accredited agency(s), whether employed, retained or otherwise engaged by the organization, will be an unbiased third-party in any services provided under this UQMP. SCO’s will not participate in any design, construction, or installation activities for projects where they also provide compliance monitoring.

UQMP Access

The Corporation will ensure its officers, staff, and SCO’s have access to a copy of this UQMP. In addition, the Corporation will ensure that any contracted accredited agency(s) providing safety codes services are provided a copy of this UQMP and any amendments.

Provision of Safety Code Officers

It shall be the policy of the Corporation to engage the services of SCO’s to meet the requirements of this UQMP. The term SCO’s throughout this QMP includes SCO’s who are employees of the corporation, hired independently (contract) by the corporation or utilized through an agency.

Only SCO’s certified to the appropriate level to provide compliance monitoring and investigation services will be engaged. Selection of specific SCO’s and assignment of duties shall be based on demonstrated knowledge and experience in relation to the duties to be performed.

SCO’s shall have authority and freedom of discretion to:

• review plans, in accordance with the SCA,

• perform inspections, in accordance with the SCA,

• issue reports which will include non-compliance items when required,

• conduct investigations, and

• issue alternative solutions / variances.

SCO’s shall not be given the authority to issue orders.

Policy for Personnel Training

It is the policy of the Corporation that all staff, SCO’s, and contract personnel performing duties under the SCA be made aware of the content of the UQMP. The Corporation will train its involved staff and SCO’s in the requirements of this UQMP, and maintain the training records on the employee file.

The Corporation will ensure that SCO’s of the Corporation attend updating seminars required by the SCC to maintain current SCO certification.

Compliance Monitoring Through Accredited Agencies

Accredited agencies retained will be provided with a copy of this UQMP and will monitor compliance with its provisions. Compliance reports will be prepared by the accredited agency(s) and forwarded to the person responsible for ensuring compliance to the provisions of this UQMP.

Contracts with agency(s) performing the work will be kept on file for audit by the Safety Codes Council or their agent.

Contracts with an accredited agency(s) shall include a statement that SCO’s have the authority and freedom of discretion related to activities as stated in the Provisions of SCO’s above.

Records: Retention and Retrieval

The Corporation will maintain a file system for all the records associated with performing the services including:

• plans, specifications, and other related documents,

• plans review reports,

• inspection reports,

• non-conformance reports,

• verifications of compliance,

• alternative solutions / variances,

• contracts with other accredited organizations, and

• related correspondence and/or other relevant information.

The Corporation will retain the files of all projects including those where an accredited agency(s) was involved, for at least three (3) years or in accordance with the Corporation’s record retention policy, whichever is greater. Such files will be available at the Corporation’s office. Records with respect to this UQMP will be filed at the area office having jurisdiction or with the person responsible for compliance to this UQMP, as applicable. Files where an accredited agency(s) was involved remain the property of the Corporation and will be returned to the Corporation within a reasonable time after completion of the services, or upon request.

The UQMP and all related records will be kept available for review by the SCC or the Council’s representative.

The Canadian Charter of Rights and Freedoms applies to all activities undertaken in the administration of this UQMP. The Freedom of Information and Protection of Privacy Act applies to all information and records relating to, created, or collected under this UQMP.

Annual Program Review

Internal audits will be carried out annually by the UQMP Manager or the Manager’s delegate. At the conclusion of the internal audit, the UQMP Manager will provide to the SCC, a letter of UQMP compliance findings, including successes, areas for improvement and the method to achieve improvement/correction. The annual review will also include a validation of facilities or sites to which this accreditation applies; as well as confirmation with respect to the facility size and staffing, specifically (1) # of staff to include contractors onsite, (2) # of sites, and (3) size of largest site (acres).

This is a mandatory requirement and failure to comply may result in cancellation or suspension of an organizations accreditation designation. All findings for the annual internal reviews are due to the Safety Codes Council no later than the last day of March of the following year that is being reviewed.

Revisions

Revisions to the UQMP will only be implemented after they have been approved by the Administrator of Accreditation and by the Corporation’s management. A record of any revisions will be maintained by the Corporation.

Schedule B

Uniform Service Delivery Standards

Section 1: Scope of Services

The Uniform Service Delivery Standards establishes responsibilities and minimum performance criteria for providing compliance monitoring services under the SCA including:

• plans examinations,

• site inspections,

• site investigations,

• alternative solutions/variances,

• verification of compliance,

• identification and follow-up of deficiencies and unsafe conditions, and

• maintaining files and records.

Section 2: Alternative Solutions / Variances

A SCO may issue an alternative solution / variance from a code or referenced standard if the SCO is of the opinion that the alternative solution / variance provides approximately equivalent or greater safety performance than that prescribed by the code or standard.

A SCO may only issue an alternative solution / variance singularly for each instance or site. A SCO does not have the authority to issue an alternative solution / variance that extends or purports to extend to more than a singular instance or site.

When an alternative solution / variance is desired, it shall be requested in writing, including all reference and research data, to a SCO. Where an alternative solution / variance is requested by persons other than employees of the Corporation (e.g. Contractor), the request shall include a signature from the UQMP Manager. Before issuing an alternative solution / variance the SCO will review and analyze the technical viability of the request. Alternative solutions / variances will be considered on a case by case basis, be issued in accordance with the SCA and SCC policy, and be in the format directed by the SCC (available on the SCC website: safetycodes.ab.ca). The SCO issuing the variance shall forward a copy of the alternative solution / variance to the SCC.

Section 3: Compliance Monitoring

3.1 General

The Corporation will monitor compliance through a program work log, plans examination (when applicable), site inspection, and follow-up inspections or verification of compliance using appropriately certified and designated SCO’s to provide compliance monitoring in accordance with the SCA.

A log shall be kept by each business unit undertaking such work. The log will contain the following information:

• project description,

• date completed,

• contact person,

• notification that infractions or deficiencies existed when inspected,

• notification when infractions or deficiencies were corrected, and

• SCO review initials.

3.2 Site Inspections / Reports

A SCO will inspect to determine if work complies with the SCA and relevant codes and standards. A SCO will, for each inspection required by this UQMP, complete an inspection report noting:

• discipline,

• business unit or department name,

• date,

• location inspected,

• stage(s) of work being inspected,

• a description of the work in place at the time of inspection,

• all observed deficiencies (any condition where the work is incomplete, or does not comply with the SCA or an associated code or regulation and in the opinion of the SCO is an unsafe condition),

• all observed situations of imminent serious danger and the action taken by the SCO to remove or reduce the danger, and

• name, signature, and designation number of the SCO conducting the inspection.

The SCO will provide a copy of the inspection report to the Corporation for each inspection.

3.3 Verification of Compliance

A SCO may, at their discretion, accept a verification of compliance (reasonable assurance provided from a third party that work complies) as follow-up to deficiencies or unsafe conditions noted on a site inspection report.

A SCO, when accepting a verification of compliance, will document the information to the file including:

• identification of the document as a verification of compliance,

• discipline,

• identification of the work it relates to,

• name and title of the person who provided the verification of compliance and how it was provided (i.e. written assurance, verbal assurance, site visit by designate, etc.),

• date accepted by the SCO, and

• signature and designation number of the SCO.

3.4 Non-Conformance/Corrective Actions

The Corporation will address all deficiencies/unsafe conditions in a timely fashion. Where the SCO and the work supervisor cannot agree on an item, the SCO may discuss the issue with the UQMP Manager. If agreement cannot be reached with the UQMP Manager, the UQMP Manager will be responsible to seek an interpretation from the Technical Administrator whose decision will be final. All non-compliant items will be resolved.

APPENDIX A: BUILDING DISCIPLINE

A building SCO will inspect work in accordance with the following category classifications. The SCO will have discretion in determining the stage, type and frequency of inspection.

The building SCO will:

• obtain any letters or schedules required to be provided by the Alberta Building Code (ABC),

• conduct a preliminary review of the construction documents to determine if professional involvement is required or if there are any potentially significant code compliance issues,

• obtain documents with the seal and signature of a registered architect and/or professional engineer(s), when required by the ABC,

• complete a review of the construction documents in accordance with the requirements of the ABC,

• prepare a Plans Review Report,

• provide the Plans Review Report to the UQMP Manager, contractor, and Corporation’s file; and if requested, to the project consultant, professionals involved, or consulting engineers, and

• provide one set of construction documents to the contractor for retention and review at the project site, and retain one set on the Corporation’s file.

Activities will be divided into three major areas:

Category 1

Day to day maintenance, alterations, and new installations not normally requiring drawings or equipment installed in accordance with existing or typical drawings; with a value of work less than $50,000.

Category 2

Projects managed by the Corporation’s personnel in accordance with a unique set of plans. This includes installation of equipment or facilities; with a value of work of $50,000 to less than $500,000.

Category 3

Large pre-engineered projects which will be managed by either the Corporation or a professional consultant, in accordance with a unique set of plans; with a value of the work being $500,000 or more.

Plans Review:

Categories 1 & 2

Plans review will normally apply. Work shall be done in accordance with the Corporation’s approved drawings and standards. Existing or typical drawings shall be updated to reflect any major changes.

Category 3

A complete set of plans shall be issued to the SCO for review, for compliance with the ABC. All non compliance items identified shall be resolved prior to construction.

Inspections:

Category 1

A log detailing day to day maintenance, alterations, and new installations will be maintained at a central location in each Area. Work in this category will be reviewed by the Corporation’s staff or designated contractors. A SCO will be retained on an annual basis to carry out a general inspection of facilities and a sampling of a minimum of 30 percent of the work completed throughout the year. The Safety Codes Officer will issue a report to the UQMP Manager, including any non-compliance items.

Non-compliance items will be corrected and documented in the appropriate Area logbook. Work not inspected by a Safety Codes Officer will be reviewed by the Corporation’s staff or designated contractors, for similar deficiencies; the corporation will identify, correct, and note any deficiencies. A SCO will re-inspect all previously noted deficiencies to insure compliance is achieved, plus 10% of work not previously inspected. The work to be re-inspected will be at the discretion of the SCO, who may re-inspect at any time. A SCO may at their discretion, accept a verification of compliance with respect to re-inspections.

In the event the SCO feels there is a critical safety issue associated with a non-compliance item, the SCO may choose to re-inspect the item prior to the next annual inspection. The SCO will inform the UQMP Manager prior to proceeding.

Category 2

A record of all Category 2 work performed in each Area, plus all required drawings and documentation, will be maintained in each Area. Work in this category will be reviewed by the Corporation’s staff or designated contractors. A SCO will be retained on an annual basis to carry out inspections on a minimum of 50 percent of all Category 2 work performed throughout the year. The SCO will issue a report to the UQMP Manager, including any non-compliance items. Non-compliance items shall be addressed using the same process as outlined in Category 1.

Category 3

All category 3 work will be inspected. The SCO will determine the frequency or number of inspections and type of inspection. Inspections required by the SCO will be determined in discussions between the Corporation, or its agent and the SCO at the point of plans review based on:

• the type of installation,

• the SCOs knowledge of the personnel involved in the work, and

• the level of project inspection by the Corporation or designated representatives.

The Safety Codes Officer will issue reports to the Project UQMP Coordinator/UQMP Manager, detailing any non-compliance items. All non-compliance items will be resolved and re-inspected.

When a facility has been completed and all outstanding non-compliance items have been resolved, it will commence to be administered under Category 1.

APPENDIX B: ELECTRICAL DISCIPLINE

Activities will be divided into three major areas:

Category 1

Day to day maintenance, alterations, and new installations not normally requiring drawings or equipment installed in accordance with existing or typical drawings.

Category 2

Projects managed by the Corporation’s personnel in accordance with a unique set of plans.

Category 3

Large pre-engineered projects which will be managed by either the Corporation, or an Engineering Consultant, in accordance with a unique set of plans. These projects generally cover construction facilities designed by professional engineers.

Plans Review:

Category 1& 2

Plans review will normally apply. Work shall be done in accordance with the Corporation’s approved drawings and standards. Existing or typical drawings shall be updated to reflect any major changes.

Category 3

A complete set of plans shall be issued to the SCO for review. The purpose of issuing plans to the SCO is primarily to prepare for inspection, however, concerns or comments the SCO may have resulting from the plans review shall be resolved.

Inspections:

Category 1

A log detailing day to day maintenance, alterations, and new installations will be maintained at a central location in each Area. Work in this category will be reviewed by the Corporation’s staff or designated contractors. A SCO will be retained on an annual basis to carry out a general inspection of facilities and a sampling of a minimum of 30 percent of work completed throughout the year. The SCO will issue a report to the UQMP Manager, including any non-compliance items.

Non-compliance items will be corrected and documented in the appropriate Area logbook. Work not inspected by a Safety Codes Officer will be reviewed by the Corporation’s staff or designated contractors, for similar deficiencies; the corporation will identify, correct, and note any deficiencies. A SCO will re-inspect all previously noted deficiencies to insure compliance is achieved, plus 10% of work not previously inspected. The work to be re-inspected will be at the discretion of the SCO, who may re-inspect at any time. A SCO may at their discretion, accept a verification of compliance with respect to re-inspections.

In the event the SCO feels there is a critical safety issue associated with a non-compliance item the SCO may choose to re-inspect the item prior to the next annual inspection. The SCO will inform the UQMP Manager prior to proceeding.

Category 2

A record of all Category 2 work performed in each Area, plus all required drawings and documentation, will be maintained in each Area. A SCO will be retained on an annual basis to carry out inspection on a minimum of 50 percent of all Category 2 work performed throughout the year. The SCO will issue a report to the UQMP Manager, including any non-compliance items. Non-compliance items shall be addressed using the same process as outlined in Category 1.

Category 3

All category 3 work will be inspected. The SCO will determine the frequency or number of inspections and type of inspections. The level of inspection required by the SCO will be determined in discussions between the Corporation, or its agent and the SCO at the point of plans review based on:

• the type if installation,

• the SCO knowledge of the personnel involved and

• the level of project inspection by the Corporation or designated representatives.

The SCO will issue reports to the Project Coordinator/UQMP Manager, detailing any non-compliance items. All non-compliance items will be resolved and re-inspected where necessary.

When a facility has been completed and all outstanding non-compliance items have been resolved, it shall commence to be administered under Category 1.

Investigations:

Investigations/incident reports of accidents to persons, fatal accidents to livestock or power line contact involving an electrical installation, electrical equipment or a fire of electrical origin or suspected electrical origin, will be conducted. These incidents will be reported to the Electrical Technical Administrator, as required by the Administrative Items Regulation. Investigations will be conducted by a SCO.

APPENDIX C: PLUMBING DISCIPLINE

Activities will be divided into two major areas:

Category 1

Minor repairs, additions, alterations, or modifications to existing facilities, in which the work consists of replacement in kind or minor changes where the value of the work is less than $10,000.00.

Category 2

This includes new facilities, and additions or modifications to facilities, in which the value of the work is $10,000.00 or more.

Plans Review:

Category 1

Generally for this type of project, design reviews will not normally apply. Site drawings shall be updated to reflect the modification or addition and made available to the SCO upon request.

Category 2

A complete set of plans shall be issued to the SCO for review. The purpose of issuing plans to the SCO is primarily to prepare for inspection. However, concerns or comments the SCO may have resulting from the plans review shall be resolved.

Inspections:

Category 1

A log detailing day to day maintenance, alterations, and new installations will be maintained at a central location in each Area. Work in this category will be reviewed by the Corporation’s staff or designated contractors. A SCO will be retained on an annual basis to carry out a general inspection of facilities and a sampling of a minimum of 30 percent of work completed throughout the year. The SCO will issue a report to the UQMP Manager, including any non-compliance items.

Non-compliance items will be corrected and documented in the appropriate Area logbook. Work not inspected by a Safety Codes Officer will be reviewed by the Corporation’s staff or designated contractors, for similar deficiencies; the corporation will identify, correct and note any deficiencies. A SCO will re-inspect all previously noted deficiencies to insure compliance is achieved, plus 10% of work not previously inspected. The work to be re-inspected will be at the discretion of the SCO, who may re-inspect at any time. A SCO may at their discretion, accept a verification of compliance with respect to re-inspections.

In the event the SCO feels there is a critical safety issue associated with a non-compliance item he may choose to re-inspect the item prior to the next annual inspection. The SCO will inform the UQMP Manager prior to proceeding.

Category 2

All Category 2 projects will be inspected.

The SCO shall report non-compliances and deficiencies, to the Corporation’s UQMP Manager, Project Co-ordinator and/or the Contractor (if applicable). Any deficiencies and/or infractions shall be corrected by the responsible party and a notice of correction shall be issued to the UQMP Manager. A SCO will re-inspect all previously noted deficiencies to insure compliance is achieved. A SCO may at their discretion, accept a verification of compliance with respect to re-inspections.

APPENDIX D: GAS DISCIPLINE

Activities will be divided into two major areas:

Category 1

A SCO will re-inspect all previously noted deficiencies to insure compliance is achieved. A SCO may at their discretion, accept a verification of compliance with respect to re-inspections.

APPENDIX E: FIRE DISCIPLINE

Fire Code Compliance Inspections

For the purpose of ensuring compliance with the SCA, a SCO may, at their discretion, carry out an inspection for any thing, process, or activity to which this Act applies. In addition, inspections will be conducted in accordance with the following schedule.

“Once every 6 months” means a specific day is set which shall apply in each 6th month for each occupancy or site to be inspected. An inspection conducted within 30 days of this set date is deemed to have met with the quantitative intent of this UQMP.

“Once every 12 months” means a specific day is set which shall apply in each 12th month for each occupancy or site to be inspected. An inspection conducted within 60 days of this set date is deemed to have met with the quantitative intent of this UQMP.

“Once every 24 months” means a specific day is set which shall apply in each 24th month for each occupancy or site to be inspected. An inspection conducted within 60 days of this set date is deemed to have met with the quantitative intent of this UQMP.

Storage Tank Systems for Flammable Liquids and Combustible Liquids

The Corporation will obtain two complete sets of construction documents signed and sealed by a professional engineer as outlined in the Alberta Fire Code. A SCO will complete a review of the construction documents to assess compliance with the requirements of the Alberta Fire Code.

Fire Investigations

Investigations will be conducted by a SCO to determine the cause, origin, and circumstance of every fire in which a person dies or suffers injury that requires professional medical attention or in which property is damaged or destroyed. The results of each investigation will be reported to the Fire Commissioner in accordance with the Administrative Items Regulation. A SCO may arrange for any additional municipal, law enforcement, agency, or other resources as required to assist in an investigation including representatives from the Fire Commissioner’s Office. In the event of a fire resulting in a death or where arson is suspected, the investigation will include immediate notification to the Alberta Fire Commissioner’s Office.

Fire Investigations will include the following information:

• location of fire,

• date of fire,

• date of investigation,

• building / property use,

• cause of fire,

• origin of fire,

• value of loss,

• name and designation number of SCO conducting the investigation,

• comments, and

• date of completion/sign off.

Fire Prevention / Pre-Planning Programs

The Corporation will ensure that a fire safety plan, an emergency pre-plan including fire preparedness and response, and a fire prevention program are in place. The Corporation will include the responding municipality’s fire department in the development of these programs and the programs will be to the satisfaction of the municipal fire department.
