Stool studies for occult blood, WBC stain, C-diff toxin screen, stool culture, rotavirus screen, and OVAS (screening test for ova and parasites in the stool). Imodium, 2 capsules by mouth after the stool specimen has been obtained, and then one capsule by mouth after each loose stool, up to a maximum of 8 capsules per day.

NTG 1/150th grain, one tablet dissolved under the tongue every 3 to 5 minutes for up to 3 doses as needed for chest pain. If the pain isn’t relieved in 15 minutes do an EKG, troponin I, CPK level, CK-MB band and call the physician.

Combivent inhaler, 2 puffs 30 seconds apart 4 times a day and as needed for breathing and coughing problems. Use the inhaler with an Aerochamber spacer.

Nebulizer treatments with one Duoneb vial (with Atrovent 0.5 mg and Albuterol 2.5 mg per nebulizer treatment 4 times a day and as needed for breathing problems.

Pulmicort Respules 0.5 mg, one vial per a separate nebulizer treatment once a day to decrease the wheezing and inflammation in the airways. Please have the patient wash out his/her mouth and gargle with water after each usage to prevent a sore mouth and throat from a fungal infection.

Flonase nasal spray, 2 sprays per nostril once a day for nasal congestion and drainage.

Ocean nasal spray, 2 sprays per nostril 4 times a day and as needed for nasal congestion and dryness.

Pulmicort Turbuinhaler 200 mcg, 2 puffs every 12 hours for wheezing and to decrease the inflammation in the airways. Please have the patient wash out his/her mouth and gargle with water after each usage to prevent a sore mouth and throat from a fungal infection.

Advair 250/50 inhaler, one puff every 12 hours for breathing problems. Please have him/her wash out his/her mouth and gargle with water after each usage to prevent a sore mouth and throat from a fungal infection.

Flovent inhaler 220 mcg, 2 puffs every 12 hours for wheezing and to decrease the inflammation in the airways. Please have the patient wash out his/her mouth and gargle with water after each usage to prevent a sore mouth and throat from a fungal infection.

Serevent Inhaler, 2 puffs 30 seconds apart every 12 hours for breathing problems.

Serevent Discus, one puff every 12 hours for breathing problems.

Flutter valve-use it for 5 minutes 4 times a day to strengthen your breathing muscles and to help you in bringing up sputum.

Albuterol inhaler, 2 puffs 30 seconds apart 4 times a day as needed for rescue of breathing problems. Please use the inhalers with a spacer such as an aerochamber spacer.

Fosomax 70 mg, one tablet by mouth one day per week for osteoporosis. Please give it on an empty stomach ½ hour before breakfast, and have the patient remain upright for ½ hour after taking it without eating or drinking anything.

Inapsine 0.25 ml IV every 4 hours (provided his EKG does not show any QT prolongation) as needed for nausea if compazine 10 mg IV q 2 to 4 hours doesn’t relieve the nausea. If the Inapsine is used, then the patient will need to be monitored on the cardiac telemetry during the administration and for 2 hours after it is given.

Actonel 35 mg, one tablet by mouth ½ hour before breakfast on Sundays for osteoporosis. Do not let the patient eat or drink and remain upright for ½ hour after taking the Actonel.

For nausea as needed use the following drugs:

Reglan 5 to 10 mg IV every 6 hours.

Zofran 4 mg IV every 6 hours.

The Reglan and the Zofran can be alternated every 3 hours to relieve nausea as needed.

Tylenol 500 mg, one tablet by mouth every 4 hours as needed for mild pain.

Milk of Magnesia, 30 ml by mouth at bedtime as needed for constipation.

Ambien 5 mg, one tablet by mouth at bedtime and may repeat X 1 if needed for sleep.

Histussin HC, one tsp by mouth every 12 hours as needed for cough and/or congestion.

Physical therapy to evaluate and treat 2 times a day for general strengthening and gait/balance training.

Zofran 4 mg IV every 6 hours as needed for nausea or vomiting.

PCA MSO4 with basal rate of 1 mg/hour with a 6-minute lock out and maximum of 20 mg over 2 hours.

Acetylcysteine 600 mg (6ml of a 100mg/ml liquid) orally every 12 hours starting on the day before the CT scan being given with IV contrast to prevent Radiocontrast Nephropathy, and continue it for 4 doses. If the CT scan must be done immediately, then give the first dose of the Acetylcysteine prior to the procedure and then continue it every 12 hour for another 3 doses after the CT scan.
