Current Concepts in Manual Therapy Management of Shoulder DisordersSEPTEMBER 23, 2017COURSE SUMMARYThis course will provide the attendee with up to date knowledge on manual therapy interventions for various shoulder disorders, using a hands-on clinical interactive approach. The session will focus on adequate screening, reassessment, dosing, and effective use of thrust and non-thrust techniques directed at the glenohumeral joint, acromioclavicular joint, scapula-thoracic complex, and the cervical and thoracic spine. Knowledge will be drawn from research the presenter has published.OBJECTIVESAttendees will recognize the most current evidence to support manual therapy interventions related to various shoulder disorders.Attendees will be familiar with an evidence based clearing and screening examination, to determine the appropriateness of manual therapy for patients with shoulder disorders.Attendees will evaluate and manage patients with musculoskeletal shoulder disorders incorporating a continuous assessment style of treatment, proper determination of manual therapy dosage, and appropriate discharge criteria.DESCRIPTIONThe evidence to support the value of physical therapy in the management of shoulder disorders is conflicting. Several systematic reviews within the last five-years provide varying levels of recommendations for the use of manual therapy. Much of this is may be due to the variation in definition of manual therapy. Whereas some look at a technique-based approach, others use more of an impairment-based approach, in which technique selection, dosage, and intensity may vary based on individual patient presentation. Approaches that focus on a wide spectrum of impairments may have more clinical value. This includes an appropriate assessment of the cervical and thoracic spine, the rib cage, the movement and control of the scapula, the glenohumeral and acromioclavicular joints, and the muscles that cross these joints. While there are many questions still to be answered, evidence-suggests that manual therapy plays a significant role in the management of shoulder dysfunction. This course will draw on the best-available evidence and cumulative experience of the presenting team who have contributed to this body of knowledge. The presenters will utilize the evidence to provide a pragmatic framework from which to approach effective management of shoulder disorders. This review will include conditions such as subacromial pain syndrome, adhesive capsulitis and acromioclavicular joint arthropathy. The course will also address the role of exercise and exercise prescription as part of an integrated manual physical therapy approach. The majority of clinical trials that study manual therapy interventions for the shoulder also utilize an exercise component. Therefore a comprehensive evidence-based approach to manual therapy should include an appropriate exercise component, along with the proper rationale to support it. There will be a strong focus on a hands-on approach with the goal of moving beyond didactic, to motor control of the hands and proper positioning.SPEAKER INFORMATIONDan Rhon, PT, DPT, DSc, OCS, FAAOMPTDr. Rhon is a clinician and active researcher for the Department of Defense in San Antonio, TX. He serves as the Director of Clinical Outcomes Research at the Center for the Intrepid, Brooke Army Medical Center. He holds graduate degrees from Baylor and Temple Universities and a postdoctoral research fellowship through the University of Utah. He is board certified in orthopaedics and a fellow in AAOMPT. He has a strong research interest in studying optimal healthcare services pathways for musculoskeletal disease, and leads funded research with grants in excess of $3 million.
