The import/export option can be used if there was a form change or if you want to start a new plan/assessment. In order to fill in and save the data on these forms you need one of the following: Adobe Acrobat Standard 7 or higher, Adobe Acrobat Professional 7 or higher. The import/export functions do not work with Adobe Reader only. When you want to Export data: Open the PDF Document from Blue Plus Care Coordination Web portal under Assessment and Care Plan formsSave the document using the Save As functionClose the documentReopen the document from your saved forms using Adobe Acrobat (not Reader)Complete the formSave or Save AsDetermine which version of Adobe you are working with (Adobe Acrobat Standard 7 or higher, Adobe Acrobat Professional 7 or higher)Export the Form Data:Using Acrobat 7 File > Form Data > Export Data from FormUsing Acrobat 8/9 Forms > Manage Form Data > Export DataUsing Acrobat 10/11Tools > Forms > More Form Options > Export Data Select where you want to save the data and hit Save **The saved data will be in .fdf version**Close the form.When you want to import data (i.e. because there was a form change, or you want to start a new plan/assessment)Open the most current version from the Blue Plus Care Coordination Web portal under Assessment and Care Plan formsSave document using the Save As functionClose the documentReopen the document from your saved forms using Adobe AcrobatImport the Form Data:Using Acrobat 7 File > Form Data > Import Data from FormUsing Acrobat 8/9 Forms > Manage Form Data > Import DataUsing Acrobat 10/11Tools > Forms > More Form Options > Import DataClick on the corresponding .fdf that you want to import Click on OpenProceed with making any changes as neededSave ................
