a) Organized and taught course to pre-doctoral dental students with Dr. Norman Treiger in Psychological and Pharmacological Factors in Pain and Anxiety Control.

b) As post-doctoral fellow (1959-61) in Periodontology and Oral Medicine, lectured on acute necrotizing ulcerative gingivitis.

c) Developed course on doctor-patient relationship with psychiatrist, Dr. Bernard Bussfield.

d) Worked with Dr. James Dunning to create the new Department of Dental Ecology; helped develop the curriculum and participated in behavioral medicine portion of the lecture program to pre- and post-doctoral students.

2) 1972-75

As the new Professor and Head of the Department of Dental Ecology, I organized and participated in teaching of 16-18 pre- and 8-10 post-doctoral students (see partial list in section II.D.1.e. below) in dental public health. The efforts were coordinated with the School of Public Health, where I held both an academic and administrative position as Assistant Director of the Center for Community Health. In addition to regularly-scheduled lectures, several seminars were held with municipal, state and federal officials involved in health care.

I have been licensed to practice psychology in Massachusetts for over forty years and dentistry in Massachusetts for fifty-eight years. During that time I have had limited clinical dental practice as a post-doctoral student at HSDM and MGH (1959-61) and then at the University Health Service. Clinical dental activities after that time were and continue to be related only to participation in various clinical research projects described elsewhere.

I currently have a small private practice as a psychologist with two principal areas of specialization: self-esteem issues related to body image; the second area is stress-related disorders, including pain management.

3. In Vivo Measurement of Anti-Inflammatory Effects of Local Anesthetic, Using Hamster Cheek Pouch Preparation

Description: I developed a reliable standardized method for quantifying the effects of pharmacologically-active, membrane-stabilizing agents on the suppression of leukocyte adherence in vivo. These results were significant in demonstrating a dose- related effect of amide local anesthetics on components of the inflammatory response.

Narrative Report

My training and experience in psychology, medicine, public health, and dentistry have been integrated into teaching, research, and patient care related to perception of pain, dysmorphia, and other stressors which influence physical and mental health.

From basic science to clinical studies, my research and teaching have ranged from endocrinology, inflammation, pain and anesthesiology, perception of orofacial morphology and function, to quality of life, including methodology of survey design, psychophysics and psychometrics, statistics, and public health again, with the training and evaluation of dentists as oral physicians to provide limited preventive primary care.

While ascending the academic and administrative ladder from Research Fellow to Professor at Harvard once and again, after being a change agent as Dean of NYU College of Dentistry and Professor in both Anesthesiology in the Medical School and Psychology in Arts and Sciences, and then serving as a Trustee at Emerson College, I continued to teach undergraduates part-time, pre and postdoctoral students, dental and medical residents, locally and internationally, many of whom have achieved outstanding reputations as clinicians, researchers, and academic leaders; in 2005 I returned to teaching undergraduates at Harvard College.

Recent public service activities include being Chairman of the Faculty of Medicine Memorial Minute Committee for Dean Paul Goldhaber and a member of the HSDM Research Training Committee.

As a consultant for BWH Anesthesia Department since 1980, I participate in weekly case conferences on psychosocial and statistical issues with residents, fellows, and staff, the most recent publication being the demonstration that patients with left-sided chronic spinal pain had more psychopathology and interference with daily activities than those with right-sided pain (Pain, 2010) and the finding of differences in patterns of brain activation (fMRI) in response to standardized noxious stimulation. Dr. Simon Gelman, Chairman of BWH Anesthesiology, in 1996 asked me to evaluate the structure, function, clinical performance, patient satisfaction, and research activities of the BWH Pain Management Center. I have recently co-authored “Gender and Ethnicity” in Principles and Practice of Pain Medicine (2016)

Other initiatives include a proposed joint HSDM/GAS DMD PhD in social sciences, and challenging dentists, in several publications noted elsewhere, with their training in medicine and surgery to expand their responsibilities to include medical screening and some preventive primary care activities as “oral physicians”, culminating in a Milton Fund Award in 2010 to train and evaluate dentists to become oral physicians, recently published in the American Journal of Public Health (2012) and a subsequent overview for publication in the Journal of Primary Prevention (2013), as well as co-author of a solicited chapter in the Second Edition (and co-editor of First Edition) of Behavioral Dentistry (2013) .

My unique accomplishments include: 1) integrating all the nontechnical aspects of dental knowledge and practice under a new department called “Dental Ecology”, which term was adopted by Harvard, the University of Minnesota, University of North Carolina, and Otago University in New Zealand, with NIH funding for training of public health dentists, while Professor and Department Chair; 2) an innovative psychophysical computer-imaging method funded by NIH for determining anthropometric bases of perceived acceptability of facial morphology, for use clinically to match the patient’s self-perception with that of the clinician to facilitate patient satisfaction with realistic treatment outcomes; 3) a new NIH-funded fiberoptic plethysmograph and later a laser Doppler method for quantitating vascular changes associated with oral infection and effectiveness and safety of local anesthetics; 4) expanding the medical responsibilities of dentists to become oral physicians; and 5) being consultant and co-investigator on a new Milton Fund grant to determine whether headache patients with left-lateralized pain have more psychopathology than those with right-sided pain.

In summary, as a psychologist and dentist I have had the opportunity to have a multifaceted and challenging career in teaching, research, and patient care involved in management of pain, body image concerns, and stress-related diseases.
