Employee Health Requirements:

• QuantiFERON Gold Plus Lab Test

o If history of positive TB BOTH a QuantiFERON Gold Lab Draw and a Chest X-Ray are to be completed

• Measles/Rubeola Titer, Mumps Titer and Rubella Titer

o Unless proof of two MMR Vaccines

• Varicella Titer

o Unless proof of two Varicella Vaccines

• Hepatitis B Titer

o Regardless of proof of Hepatitis B Series, we need a titer to show that immunity was gained after series was completed.

• Flu Vaccine or Declination (during flu season)

• Tetanus, Diphtheria and Pertussis Vaccine (Tdap)

• Mask Fitting

• ReadySet Account (Our Employee Health System for tracking records – see instructions below)

What to expect:

• Must schedule an appt. with Employee Health, please plan for 30-45 minutes for appt. and parent must be present if student is under the age of 18

• Must come prepared with all survey’s completed and

• Valid photo ID (School ID ok) and

• Drink at least 16 oz of water to prepare for drug screening

Follow the instructions to create a username and password and complete your Health Assessment Surveys.

o Organizational 4-digit Code: 1733

o Program Type:

▪ New Hire

o Population type: Choose appropriate selection….

▪ Student

Complete the Health Assessment Surveys. 

All survey statuses will have a green checkmark under status when you are finished.

**Survey’s must be completed by parent if student is under 18**
