Kaiser Permanente: Call first to set up an order for a TB test or Hepatitis B vaccination. For employees who do not have a Kaiser Permanente Health Record Number then they can set up an account for you. They will require your name, date of birth, social security number (to determine if you have had a previous health record number with them) and mother's maiden name to complete the process. For the Portland area call 503-571-3366 and for the Vancouver area call 1-888-414-3531.
NOTE: All TB tests require two visits- one for the test and one to read the test, normally 2-3 days later.
