BENEFITS PACKAGE

This document is a brief description of agency benefits for full time employees.

Some benefits are available for employees working part-time.

Effective Date of Insurance Coverage is the 1st day of the month following 30 days of employment

| |WorkAbilities health insurance is provided through HealthPartners. Coverage consists of a|

|HEALTH INSURANCE |high-deductible insurance plan with an accompanying health savings account (HDHP w/HSA). |

| |Family coverage is available, and is the financial responsibility of the employee. |

|High Deductible Health Plan (HDHP) with Health Savings |WorkAbilities pays $357.82/month of the plan premium and contributes $143 to the |

|Account (HSA) |employee’s HSA. The employee is responsible for $187.32/month. The annual deductible is|

| |$3500. |

| |WorkAbilities dental insurance is provided through Delta Dental of MN. |

|DENTAL INSURANCE |WorkAbilities pays 100% of the monthly premium for the employee. Family coverage is |

| |available and is the financial responsibility of the employee. |

| |WorkAbilities life insurance is provided through Unum Insurance Co. It provides |

|LIFE INSURANCE |employees $10,000 benefit term life insurance at no cost to the employee. |

| |WorkAbilities provides long term disability insurance through Unum Insurance Co. at no |

|LONG TERM DISABILITY |cost to the employee. |

|SHORT TERM DISABILITY*** |***Employees may also purchase short term disability coverage at their own expense from |

| |Colonial Life Insurance Co. |

| |WorkAbilities offers its employees the option of participating in a 403B tax-deferred |

|403 B RETIREMENT PLAN |annuity retirement program. WorkAbilities may make a discretionary matching contribution|

| |to the plan at the end of the year. |

| |WorkAbilities offers its employees the option of participating in a Section 125 Flex |

|FLEX BENEFIT PLAN |Benefit Plan (Flex Spending Account). This plan allows participating employees to pay |

|(FLEX SAVINGS ACCOUNT) |health care and dependent care expenses with pre-tax dollars, thus making them more |

| |affordable. Employees can also pay their group insurance premiums through this plan, |

| |with pre-tax dollars. |

| |WorkAbilities’ employees earn the following: |

|PAID TIME OFF |Years 1 - 2 15 days of paid time off |

| |Years 3 – 10 18 days of paid time off |

| |Years 11 – 20 21 days of paid time off |

| |Years 21 + 24 days of paid time off |

| |WorkAbilities provides 8 paid holidays per year: |

| |New Year’s Day Martin Luther King, Jr. Day Memorial Day July 4th Labor Day |

|HOLIDAY |Thanksgiving & day after New Year’s Eve |

| |Employees are paid every two weeks on Friday. Employees may choose either paper checks or|

|PAY DAYS |direct deposit. |

| |WorkAbilities, Inc. will reimburse full time employees 50% of tuition for one approved |

|TUITION REIMBURSEMENT |and successfully completed college or continuing education course per semester. |

| |Paid lunch break of 45 minutes, free parking, free Notary Public services, smoke-free |

|OTHER BENEFITS |work environment. |
